Arterialsti?nessisincreasedintype1diabetes(T1D),beforeanyclinicalcomplicationsofthediseaseareevident.Theaimofthe presentpaperwastoinvestigatetheassociationbetweencardiacautonomicfunctionandarterialsti?nessinacohortofyoung T1D patients, without history of hypertension and any evidence of macrovascular and/or renal disease. Large artery sti?ness wasassessedbymeasurementofcarotid-femoralpulsewavevelocity(PWV).Cardiacautonomicfunctionwasassessedbythe cardiovasculartestsproposedbyEwingandClarke.Patientswithahighcardiacautonomicneuropathyscore(≥4)hadsigni?cantly higherPWVthanthosewith alowscore(0-1).Anegative,heart rate-independent,correlationbetweenPWVandheart rate variationduringrespirationwasobserved(r=?0.533,P0.001).Inmultivariableanalysis,E/Iindexwasthestrongestcorrelate ofPWV(β-coe?cient=?0.326, P =0.002).Cardiacparasympatheticfunctionisastrongpredictoroflargearterialsti?ness,in youngT1Dpatientsfreeofmacrovascularandrenalcomplications. 1.Introduction may occur within two years of the diagnosis in patients withT1D[16].Reducedcardiovascularautonomicfunction has been associated with increased total mortality and cardiovascularmorbidityaswellasmortality[17,18].
