Methods:Seventy-fourconsecutivepatientswithpainfulpolyneuropathyandnormalnerveconductionstudies(NCS)were definedasSFSNcasesorcontrolsbydistalIENFD,5.4and$5.4fibers/mm,respectively.Diagnosticperformanceofsmall fiber[cooling(CDT)andheatperception(HP)thresholds,axonreflex-mediatedneurogenicvasodilatation]andlargefiber functiontests[vibrationperceptionthresholds(VPT)andsuralnerveconductionparameters]weredeterminedbyreceiver operating-characteristic(ROC)curveanalyses. Results:The26(35%)SFSNcaseshadmeanIENFD3.361.7fibers/mmandthe48(65%)controls9.962.9fibers/mm.Male gender(p=0.02)andolderage(p=0.02)wereassociatedwithSFSNcasescomparedtocontrols.VPTwerehigherandCDT lowerinSFSNcases,butthelargestmagnitudeofdifferenceswasobservedforsuralnerveamplitude.Ithadthegreatest areaundertheROCcurve(0.75)comparedtoallothertests(p,0.001forallcomparisons)andtheoptimalthresholdvalue of#12mVdefinedSFSNcaseswith80%sensitivityand72%specificity. Conclusion:InpatientspresentingwithpolyneuropathymanifestationsandnormalNCS,thoughsmallfiberfunctiontests wereintuitivelyconsideredthebestalternativemeasurestopredictreducedIENFD,theirdiagnosticperformancewaspoor. Instead,novelthresholdvalueswithinthenormalrangeforlargefibertestsshouldbeconsideredasanalternativestrategy toselectsubjectsforskinbiopsyindiagnosticprotocolsforSFSN.
