Randomizedcontrolledtrialsofprogestogenstopreventpretermbirthintwinpregnancieshaveshownnoclear benefits.However,individualstudieshavenothadsufficientpowertoevaluatepotentialbenefitsinwomenat particularhighriskofearlydelivery(forexample,womenwithapreviouspretermbirthorshortcervix)orto determineadverseeffectsforrareoutcomessuchasintrauterinedeath. Methods/design:Weproposeanindividualparticipantdatameta-analysisofhighqualityrandomized,double- blind,placebo-controlledtrialsofprogestogentreatmentinwomenwithatwinpregnancy.Theprimaryoutcome willbeadverseperinataloutcome(acompositemeasureofperinatalmortalityandsignificantneonatalmorbidity). Missingdatawillbeimputedwithineachoriginalstudy,beforedataoftheindividualstudiesarepooled.The effectsof17-hydroxyprogesteronecaproateorvaginalprogesteronetreatmentinwomenwithtwinpregnancies willbeestimatedbymeansofarandomeffectslog-binomialmodel.Analyseswillbeadjustedforvariablesusedin stratifiedrandomizationasappropriate.Pre-specifiedsubgroupanalysiswillbeperformedtoexploretheeffectof progestogentreatmentinhigh-riskgroups.
