Casepresentation:Wedescribedacaseofa15-year-oldpatientwithahistoryofacutepharyngodiniaearly followedbyimprovisefeverandaprogressiveformationofadiffuseorbitaledema,cornealhyperaemia,diplopia andseveredecreaseofvisualacuity. Thepatientwassurgicallytreatedwithfunctionalendoscopicsinussurgery(FESS)aftertheresponseofa maxillofacialcomputedtomographyscansthatshowedapansinusitiscomplicatedbyaleftorbitalcellulites. NumerouscoloniesofStreptococcuspyogeneswerefoundinthesamplesofpusandanantibiotictherapywith meropenemwasinitiatedonthebasisofthesensitivitytesttoantibiotics.Thepatientwasfinallydischargedwith diagnosisofleftorbitalcelluliteswithperiorbitalabscess,endophtalmitisandacutepansinusitisasaconsequence ofstreptococcalpharyngitis. Conclusion:Thecasehighlightsthepossibleunusualcomplicationofagroup-Astreptococcalpharyingitisina immunocompetentchildandtheneedingofapromptsurgicalandmedicalapproachtowardthemaxillofacial complicationsassociatedtotheinfection.
