Methods:UsingKellgren-Lawrence(K/L)gradingearlyradiographickneeOA(K/L2)wasdetectedin16of46 patients.Thesegrades(K/L1isnoOAandK/L2isearlyOA)weredividedintotwogroupsaccordingtothepresence orabsenceofpersistentkneepain.Sera(s)andurines(u)wereanalysedwithbiomarkersforcartilagecollagen cleavage(sC2CanduCTX-II)andsynthesis(sCPII),boneresorption(uNTx)andsynovitis(hyaluronicacid:sHA). Results:sCPIIdecreasedandsC2C/sCPII,uCTX-II/sCPIIandsHAincreasedwithonsetofOA(K/L2versusK/L1) irrespectiveofjointpain.Incontrast,sC2CanduCTX-IIremainedunchangedinearlyOApatients.Ofthepatients withK/Lgrades1and2sC2C,sCPII,sHA,uNTXanduCTX-IIwereallsignificantlyincreasedinpatientswithknee painindependentofgrade.AmongtheK/Lgrade2subjects,onlyuCTX-IIanduCTX-II/sCPIIwereincreasedin thosewithkneepain.Ingrade1patientsbothsC2CandsCPIIwereincreasedinthosewithkneepain.Nosuch gradespecificchangeswereseenfortheotherbiomarkersincludingsHA. Conclusions:Theseresultssuggestthatchangesincartilagematrixturnoverdetectedbymolecularbiomarkers mayreflectearlychangesincartilagestructurethataccountdirectlyorindirectlyforkneepain.AlsoK/Lgrade1 patientswithkneepainexhibitbiomarkerfeaturesofearlyOA.
