Methods:AspontaneouspainpatterninFMwasrecordedin30FMpatientsand30healthysubjectsservedas controls.Localandreferredpainpatternsinducedfromactive(patients)andlatent(controls)MTPswererecorded followingmanualstimulation.TheexistenceofMTPswasconfirmedbyintramuscularelectromyographical registrationofspontaneouselectricalactivity. Results:LocalandreferredpainareasinducedfromkeyactiveMTPsinFMwerelargerthanpainareasfromlatent MTPsinhealthycontrols(P0.001),butweresimilartotheoverallspontaneousFMpainareainFM(P0.05). TheinducedpainareawaspositivelyassociatedwithcurrentspontaneouspainintensityinFM(P0.01).The locationsofkeyactiveMTPsinFMpatientswerefoundtohavelatentMTPsinhealthysubjects.Themuscles containingkeyactiveMTPsinFMareoftenobservedinthemusclesofextensordigitorum,trapezius,infraspinatus intheupperpartofthebodyandofquadratuslumborum,gluteusmediusinthelowerpartofthebody. Conclusions:TheoverallspontaneousFMpainpatterncanbereproducedbymechanicalstimulationofactive MTPslocatedindifferentmuscles,suggestingthatfibromyalgiapainislargelycomposedofpainarisingfrom musclepainandspasm.TargetingactiveMTPsandrelatedperpetuatingfactorsmaybeanimportantstrategyin FMpaincontrol.
