Methods:Fiftyhealthycontrolsand94patientswithacutesevereTBIwereincluded.Plasmasampleswere obtainedonadmissionandatdays1,2,3,5and7afterTBI.Itsconcentrationwasmeasuredbyenzyme-linked immunosorbentassay. Results:Twenty-sixpatients(27.7%)diedfromTBIwithin1month.AfterTBI,plasmaresistinlevelinpatients increasedduringthe6-hourperiodimmediatelyafterTBI,peakedwithin24hours,plateauedatday2,decreased graduallythereafterandwassubstantiallyhigherthanthatinhealthycontrolsduringthe7-dayperiod.Aforward stepwiselogisticregressionselectedplasmaresistinlevel(oddsratio,1.107;95%confidenceinterval,1.014-1.208; P=0.023)asanindependentpredictorfor1-monthmortalityofpatients.Amultivariatelinearregressionshowed thatplasmaresistinlevelwasnegativelyassociatedwithGlasgowComaScalescore(t=-6.567,P0.001).A receiveroperatingcharacteristiccurveidentifiedplasmaresistincutofflevel(30.8ng/mL)thatpredicted1-month mortalitywiththeoptimalsensitivity(84.6%)andspecificity(75.0%)values(areaundercurve,0.854;95%confidence interval,0.766-0.918;P0.001).
