Methods:Acasecontrolstudywasconductedamong402children,comprising193normal-weightand209 overweight/obese.Weight,height,waistcircumference(WC)andbodycompositionweremeasured,andWHO (2007)growthreferencewasusedtocategorisechildrenintothetwoweightgroups.Bloodpressure(BP)was taken,andbloodwasdrawnafteranovernightfasttodeterminefastingbloodglucose(FBG)andfulllipidprofile, includingtriglycerides(TG),high-densitylipoproteincholesterol(HDL-C),low-densitylipoproteincholesterol(LDL-C) andtotalcholesterol(TC).InternationalDiabetesFederation(2007)criteriaforchildrenwereusedtoidentify metabolicsyndrome. Results:Participantscomprised60.9%(n=245)Malay,30.9%(n=124)Chineseand8.2%(n=33)Indian.Overweight/ obesechildrenshowedsignificantlypoorerbiochemicalprofile,higherbodyfatpercentageandanthropometric characteristicscomparedtothenormal-weightgroup.Amongthemetabolicriskfactors,WC≥90thpercentilewas foundtohavethehighestodds(OR=189.0;95%CI70.8,504.8),followedbyHDL-C≤1.03mmol/L(OR=5.0;95%CI2.4, 11.1)andhighBP(OR=4.2;95%CI1.3,18.7).Metabolicsyndromewasfoundin5.3%oftheoverweight/obesechildren butnoneofthenormal-weightchildren(p0.01).Overweight/obesechildrenhadhigherodds(OR=16.3;95%CI2.2, 461.1)ofdevelopingthemetabolicsyndromecomparedtonormal-weightchildren.Binarylogisticregressionshowed nosignificantassociationbetweenage,genderandfamilyhistoryofcommunicablediseaseswiththe
