Collinsetal.InfectiousDiseasesofpoverty2012,1:8 /content/1/1/8 OPINION OpenAccess Schistosomiasiscontrolandthehealthsystem inP.R.China CharlesCollins1,JingXu2andShenglanTang3* Abstract OverthelastsixtyyearsadvanceshavebeenmadeinthecontrolofschistosomiasisinP.R.China.Thereare, however,difficultchallengesstilltobemet.Thispaperlooksattheextenttowhichthehealthsystemoffersa positiveenvironmentforthecontrolofthedisease.Itstartsbytracingthreephasesinschistosomiasiscontrol: diseaseeliminationstrategythroughsnailcontrol(1950s-early1980s);morbiditycontrolstrategybasedon chemotherapy(mid1980sto2003);integratedcontrolstrategy(2004+).Eachoneofthesephasestookplacein distinctpolicy-makingenvironments.Thepaperpartlydrawsonthesephasestosetoutfiveissuesofdisease controlanddiscussestheminthecontextofthehealthsystemanditsrecenttrends.Thesecoverthe policy-makingprocess,intersectoralactionforhealth,equityandaccesstohealthservices,fundingforpublicgoods andexternalities,andstrengtheningresourcemanagementandplanning.Theseissuesformthebasisofanagenda forintegratingresearchandcapacitystrengtheningintheChinesehealthsystemwithaviewtocreatingamore positiveenablingenvironmentforschistosomiasiscontrol.Insodoingitisimportanttoemphasizetheroleand integrityofthepublicsectoragainstitscommercialization,theunderlyingvalueofequity,asystemswide perspective,andtheroleofadvocacy.
