Objectives:TocharacterisealargecohortofresidentsinRomeaccordingtodifferentmeasuresoftrafficexposure, socioeconomicposition(SEP),andbaselinehealthstatus. Methods:ResidentsofRomeinOctober2001wereselected.Individualandarea-basedSEPindiceswereavailable. GISwasusedtoobtaintrafficindicatorsatresidentialaddresses:distancefromHTR(=10,000vehicles/day), lengthofHTR,averagedailytrafficcount,andtrafficdensitywithin150metersofhome.Hospitalisationsinthe 5-yearperiodbeforeenrolmentwereusedtocharacterisehealthstatus.Logisticandlinearregressionanalyses estimatedtheassociationbetweentrafficexposureandsocio-demographiccharacteristics. Results:Weselected1,898,898subjectswithcompleteSEPinformationandGIStrafficindicators.Atotalof320,913 individuals(17%)livedwithin50metersofanHTR,and14%livedbetween50and100meters.Theseproportions werehigheramong75+year-oldsubjects.Overall,alltrafficindicatorsweredirectlyassociatedwithSEP,with peoplelivinginhighormediumSEPareasorwithauniversitydegreemorelikelytobeexposedtotrafficthan peoplelivinginlowSEPareasorwithalowlevelofeducation.However,aneffectmodificationbyareaof residencewithinthecitywasseenandtheassociationbetweentrafficandSEPwasreversedinthecitycentre. Conclusions:AlargesectionofthepopulationisexposedtotrafficinRome.Elderlypeopleandthoselivingin areasofhighandmediumSEPtendtobemoreexposed.Thesefindingsarerelatedtothehistoricalstratification ofthepopulationwithinthecityaccordingtoageandsocioeconomicstatus.
