HolmesandJosephGlobalizationandHealth2011,7:43 /content/7/1/43 DEBATE OpenAccess Socialparticipationandhealthyageing: aneglected,significantprotectivefactorfor chronicnoncommunicableconditions WendyRHolmes1*andJenniferJoseph2 Abstract Background:Lowandmiddleincomecountriesareageingatamuchfasterratethanrichercountries,especially inAsia.Thisishappeningatatimeofglobalisation,migration,urbanisation,andsmallerfamilies.Olderpeople makesignificantcontributionstotheirfamiliesandcommunities,butthisisoftenunderminedbychronicdisease andpreventabledisability.Socialparticipationcanhelptoprotectagainstmorbidityandmortality.Wearguethat socialparticipationdeservesmuchgreaterattentionasaprotectivefactor,andthatolderpeoplecanplayauseful roleinthepreventionandmanagementofchronicconditions.Wepresent,asanexample,alow-cost,sustainable strategythathasincreasedsocialparticipationamongeldersinSriLanka. Discussion:Currentinternationalpolicyinitiativestoaddresstheincreasingprevalenceofnon-communicable chronicdiseasesarefocusedoncardiovasculardisease,diabetes,respiratorydiseaseandcancers,responsiblefor muchprematuremortality.Interventionstomodifytheirsharedriskfactorsofhighsaltandfatdiets,inactivity, smokingandalcoholuseareadvocated.Butolderpeoplealsosufferchronicconditionsthatprimarilyaffectquality oflife,andhaveawiderrangeofriskfactors.Thereisstrongepidemiologicalandphysiologicalevidencethat socialisolation,inparticular,isasimportantariskfactorforchronicdiseasesasthe‘lifestyle’riskfactors,yetitis currentlyneglected.Thereareusefulexperiencesofinexpensiveandsustainablestrategiestoimprovesocial participationamongolderpeopleinlowandlowermiddleincomecountries.OurexperiencewithformingElders’ ClubswithretiredteaestateworkersinSriLankasuggestsmanybenefits,includingsocialsupportand participation,inter-generationalcontact,acollectivevoice,andfacilitatedaccesstohealthpromotionactivities,and tohealthcareandsocialwelfareservices.
