Lederetal.JournalofOphthalmicInflammationandInfection2013,3:30 /content/3/1/30 ORIGINAL RESEARCH OpenAccess Ultra-wide-fieldretinalimaginginthe managementofnon-infectiousretinalvasculitis HenryALeder1,2,JohnPCampbell1,3,YasirJSepah1,TheresaGan4,JamesPDunn4,ElhamHatef1,BrianCho1, MohamedIbrahim1,MillenaBittencourt1,RoomasaChanna1,DianaVDo1,5andQuanDongNguyen1,5* Abstract Background:Thepurposeofthisstudyistodescribeandquantifythebenefitofultra-wide-fieldimagingand fluoresceinangiography(FA)inthemanagementofnon-infectiousretinalvasculitis.Inthisprospective observationalcohortseries,patientswithnon-infectiousretinalvasculitiswereevaluatedandenrolledbyfour investigatorsfromtheDivisionsofRetinaandOcularImmunologyattheWilmerEyeInstitute.Ineachpatient, diseaseactivityandtheneedformanagementchangeswereassessed,basedonclinicalexaminationwithor withoutstandard(60°)imagingandthenwiththeadditionofultra-wide-fieldpseudo-colorscanninglaser ophthalmoscope(SLO)imagesandFAusingtheOptosultra-wide-fieldSLO(OptosPanoramic200MA?,OptosPLC, Dunfermline,Scotland,UK).Astandardizedquestionnairewascompletedbyeachinvestigatoratthetimeofthe clinicalevaluation. Theprimaryoutcomewasthepercentageofpatientswhosemanagementwaschangedbyclinicalexamination andstandardFA,comparedwithclinicalexaminationplusultra-wide-fieldimaging.Thesecondaryoutcomewasthe percentageofpatientswhosediseasewasdeterminedtobeactivebasedoneachmodality. Results:Seventy-onevisitsfrom23patientswerereviewedandanalyzed.Basedonexaminationplusultra-wide-field imagingandultra-wide-fieldangiography,diseaseactivitywasdetectedin48/71(68%)comparedwith32/71(45%) basedonexaminationandstandardFA(P=0.0095).Basedontheclinicalexaminationalone,thedecisiontoalter managementwasmadein4of71visits(6%),andanadditional3of71(4%)basedonsimulatedstandardFA.The additionofultra-wide-fieldSLOpseudo-colorimagesalteredmanagementinanadditional10/71visits(14%),and 36/71(51%)withtheadditionofultra-wide-fieldFA.
