药物不良反应杂志204年 2月第 6卷第6期 ADRj，Dece ber20 4，Vol 6 No6 3·79 · ·病例报告· 碘海醇注射液致过敏反应和严重血小板减少 及凝血功能异常 王杰松 张朝利 费军 谢雅君 关笑婵 吴久鸿 【摘要】 1 例63岁女性行冠状动脉CT增强检查，静脉注射造影剂碘海醇80ml（350mg/ml）。 CT检查结束留观6min时，患者出现手掌瘙痒、双眼结膜充血水肿 周身大汗、黑曚、恶心，呕吐少量胃 内容物，随后出现唇紫、颜面水肿、四肢凉、皮肤花斑、注射部位出血。血气分析：血氧饱和度0.83，氧分 9 压47mmH （1mmHg=0.133kPa）。距CT检查结束近1h，实验室检查示血小板计数19×10/L ，凝血 酶原时间（PT）18s，凝血酶时间（TT）6s。给予抗过敏、补充凝血因子 纠正酸中毒和电解质紊乱等治 9 患者身体多处出现大片瘀斑，体温 疗。距CT检查结束约7.5h，患者血小板计数8×10/L。次日， 9/L。继续行对症和支持治疗， 38.1℃，血小板计数6×10 患者情况逐渐好转，第4天体温恢复正常，颜面 水肿消失，瘀斑明显减轻；第6天血小板计数和凝血功能各项指标均恢复正常。 【关键词】 碘海醇； 过敏反应； 血小板减少； 血液凝固 Anaphylaxis，seriousthrombocytopenia，andcoagulationdisordersinducedbyiohexolinjection Wang * * Jiesong ，ZhangChaoli，FeiJun，XieYajun，GuanXiaochan，WuJiuhong.
