* Dose of therapy: This is a very important issue and not one that is well recognized at this point. There is a single, randomized, controlled trial that was published by Claudio Ronco last year in The Lancet where he randomized patients to receive three doses of therapy. This was CVVH using post-filter replacement. The doses of therapy were 20 ml/kg/hr, 35 ml/kg/hr, and 45 ml/kg/hr. What he observed was that survival was significantly higher at the two higher doses of therapy, 57 and 58 percent survival as opposed to 41 percent survival in the lower dose of therapy. This is certainly not a de
