山东医药2007年第47卷第36期 黏结式上颌快速扩弓联合前方牵引治疗 大龄骨性Ⅲ类错殆临床观察 孔凡芝 ，佘 鹏 ，王玉玲 ，丁双英 ，江玉凤 ，经海永 ，王春玲 (1江苏大学附属人民医院，江苏镇江212002；2山东省千佛山医院； 3山东大学口腔医学院) [摘要] 目的 观察粘结式上颌快速扩弓联合前方牵引治疗大龄骨性Ⅲ类错黯的疗效。方法 分别选取6 ～ 9岁和10～15岁各30例骨性安氏Ⅲ类错殆患者，先行粘结式上颌快速扩弓治疗，待上下颌覆盖达3～4 mm时， 给予上颌前方牵引治疗，牵引约半年之后去除矫治器。结果 所有患者上颌骨及上切牙前移，使面中部及上唇前 移，下颌骨及下切牙后退，使颏部及下唇后移，10～15岁患者部分参数变化程度较小，但与6—9岁者相比差异无统 计学意义。结论 粘结式上颌快速扩弓结合上颌前方牵引治疗10～15岁骨性Ⅲ类错殆 依然可以取得良好效果。 [关键词] 错殆畸形；上颌前方牵引；快速扩弓 [中图分类号] R783．5 [文献标识码] A [文章编号] 1002-266X(2007)36-0030-03 Clinical observations of patients aged 1 0-1 5 years with skeletal class lIl malocclusion treated with a bonded maxillary expansion and maxilla protraction KONG Fan-zhi ，SHE Peng，WANG Yu-ling，DING Shuang—ying ， JIANG Yu-feng，JING Hai-yo ，WANG Chun-ling (1The First Affiliated People Hospital ofJiangsu Unive~ity，Zhenjiang 212002，P．R．China) Abstract：[Objective]To observe the results of patients aged 10～15 years with skeletal class III malocclusion trea- ted with a bonded m~Uary expansion and maxilla protraction．[Methods]Thirty cases of patients aged 6～9 years with skeletal class III malocclusion and thiny cases of patients aged 10～15 years with skeletal class m malocclusion were se· lected．Firstly，these patients were treated with a bonded maxillary expansion．When the anterior teeth overbite was to 3～ 4mm，the patients received the maxilla protraction for six months．[Results]The maxillary and maxillary incisors of all pa- tients were moved to the anterior．The midface and upper lip were moved to the anterior．Whereas the mandible rotated and the mandibular incisors moved posteriorly，the chin and l ower lip were moved to the posterior．The changes of some parame- ters in patients aged 10～15 years were less than the changes in patients aged 6～9 years．But the differences have nosta·
