提 要 目的:介绍经内外踝截骨入路手术治疗复杂的距骨骨折的方法,探讨此手术入路 近期疗效。 方法:2009年2月至2012年2月，选取山东省中医院符合标准的复杂距骨骨折12例, 其中男8例, 女4例, 年龄18～44岁,平均年龄是27.6岁；高空坠落伤8例, 车祸伤3例， 重物砸伤1例；左侧距骨骨折8例,右侧距骨骨折4例；合并踝部骨折3例,开放性骨折脱 位1例,均应用经内外踝截骨入路行手术治疗。 结果:全部病例均获得随访，随访时间8个月～3年,平均时间2年。根据患者主诉、 足跟及踝关节的外形、功能及影像来评价此手术入路的临床疗效。按Hawkins疗效标 准: 优6例,良3例，可2例,差1例,优良率为75%,本组病例均没有出现距骨体坏死。 结论:经内外踝截骨手术入路是治疗复杂距骨骨折的良好方法。 关键词 距骨，骨折，截骨入路，手术治疗，疗效 The medial and lateral malleolus osteotomy approach for treatment of complex talar fracture Speciality:Orthopaedics of TCM Author:Xue Xiao liang Tutor:sun lu Abstract Objective: To introduce the internal or external malleolus osteotomy approach operation in treatment of complex fracture of the talus method, discusses the operation approach and curative effect in the near future.Method: 2009February to2012February,12 cases were complicated talus fracture, male 8, female4, age 18~ 44year old, average age is 27.6 years old; high fall injury in 8 cases,3 cases of traffic accident injuries, heavy injured in 1 cases; the left talus fracture in 8 cases, right talus fractures in 4 patients with ankle fractures in3; cases, open fracture and dislocation in 1 cases, were applied by internal or external malleolus osteotomy approach operation.Results: All patients were followed up, followed up for 8 months to3 years, average time of 2years. According to patients complaints, the heel and ankle shape, function and imaging to evaluate the clinical efficacy of operation approach. According to Hawkins standards of efficacy: excellent in 6 cases, good in 3 cases, in 2 cases, poor in 1 cases, the excellent and good rate was 75%, all cases were no body of talus necrosis.
