经舒饮治疗寒凝血瘀型原发性痛经的临床探究.pdfVIP

提 要 目的：观察经舒饮方治疗寒凝血瘀型原发性痛经的临床疗效，初步探讨其镇痛的 作用机理。方法：将门诊就诊的 60 例原发性痛经属于寒凝血瘀型的患者随机分为治 疗组和对照组各 30 例，治疗组服用经舒饮方煎剂，对照组服用少腹逐瘀颗粒。于月 经前 7 天开始服药，共服药 10 天，一个疗程为三个月经周期。用药一个疗程后，对 两组治疗前后的痛经疗效及中医症状改善情况进行疗效评价；对比观察治疗组治疗前 后子宫动脉血流情况、血液流变学指标及经血前列腺素 PGF2 α含量变化。结果：治疗 组与对照组组间比较，痛经疗效差异有显著性（P ＜0.05 ）；中医证候疗效差异有显著 性（P ＜0.05 ）；治疗组治疗前后子宫动脉搏动指数（PI ）、阻力指数（RI ）及收缩期 峰值/舒张期峰值（A/B ）均明显下降，盆腔血流量较前明显增加，差异有显著性（P ＜0.05 或 P ＜0.01 ）；治疗组治疗前后血液流变学各项指标有明显降低，差异有显著性 （P ＜0.05 或 P ＜0.01 ）；治疗组治疗后经血前列腺素 PGF2 α含量有显著降低，差异有 显著性（P ＜0.01 ）。结论：经舒饮方治疗寒凝血瘀型原发性痛经，临床运用疗效确切， 安全性高。其作用机制可能是通过增加盆腔血流灌注，改善血液流变性，降低经血 PGF2 α含量，缓解子宫平滑肌过度收缩等作用实现的。 关键词 经舒饮方；寒凝血瘀；原发性痛经；止痛机理 Clinical Investigations on the Treatment of Primary Dysmenorrhea of Congealing Cold and Blood Stasis with Jingshuyin Formula Speciality：Gynecology of Traditional Chinese Medicine Author ：Suo Xiaoying Tutor：Professor Liu Jinxing Abstract Objective：To observe the clinical effect and safety of Jingshuyin(JSY)Formula on treating Primary Dysmenorrhea (PD) of congealing cold and blood stasis,and to explore its mechanism of action.Method：Choose 60 patients who belong to PD of congealing cold and blood stasis.Divided them randomly into test group(30 cases,treated with JSY Formula),and control group(30 cases,treated with Shaofuzhuyu Cranule).Both groups started at 7 days before the menstrual period and lasted 10 days,a course contained 3 menstrual cycles.Before and after treatment,the therapeutic effects on PD,TCM symptoms,the change of the uterine blood circulation,the blood rheology,and the PGF2 α in the menstrual blood would be compared.Results ：The therapeutic effects on PD has difference between 2 groups(P＜0.05).The TCM symptom of the test group and contro
