Abstract Objective：To get a systematic overview of the clinical efficacy and safety of apecitabine combined with radiotherapy on advanced rectal cancer. Methods：The computer searched MEDLINE, the China Biological Medicine Disc (CBM), the China Knowledge Infrastructure (CNKI) and the Wan fang database, in accordance with evidence-based medicine (Cochrane) systematic overview methods, review the quality of the included studies one by one and extract valid data, using RevMan 5.3 software with Meta-analysis. Results： trials and 1876 patients in total were analyzed in this meta-analysis. The results of Meta-analysis showed that the experimental group had higher overall response rate, radical resection rate, anal-saving rate, one- and two-year survival rates, as compared with the control group (P0.01). The anastomotic leakage rate and local recurrence (LR) rate after treatment in the experimental group was significant lower than that in the control group (P0.01). Furthermore, the adverse reactions were also higher in the experimental group than those in the control group. No difference in remission rate of pain was found between the experimental and control group (P0.01).
