心电图及心向量图诊断心房及心室肥大对比分析 　 作者：蔡晓虹，柳飞强，宋艳蝶，蔡富锦，潘翠霞 【关键词】 心室肥大 ［摘要］ 目的：探讨心电图（ECG）与心向量图（VCG）联合应用诊断心房、心室肥大的临床价值。方法：应用心电工作站对观察组300例及对照组300例进行单独ECG及VCG、联合ECG和VCG诊断心房、心室肥大，分别计算3种方法的敏感性、特异性、准确性并进行对比。结果：ECG和VCG联用对左心房、右心房、左心室、右心室、双心房、双心室肥大诊断的敏感性及准确性最高，ECG单独应用及ECG和VCG联用对右心房及双心房肥大的诊断敏感性相同，VCG单独应用和ECG和VCG联用对右心室肥大的诊断敏感性相同；3种方法对心房、心室肥大诊断的特异性相近；ECG和VCG联用诊断心房、心室肥大时结合临床资料可提高准确性。结论：ECG和VCG联用诊断心房、心室肥大可弥补心电图的不足。 　　［关键词］ ECG；VCG；诊断；心房肥大；心室肥大 　　Contrastive Analysis on the Electrocardiogram and Vectorcardiogram Diagnosing the Atrial or Ventricular Hypertrophy 　　Abstract：Objective To probe the clinical value of the ECG plus VCG diagnosing the atrial or ventricular hypertrophy.Methods By establishing an ECG work station,the atrial or ventricular hypertrophy patients of 300 cases in observation group and 300 cases in control group were respectively diagnosed by only ECG or VCG or ECG plus VCG. The sensibilities, specificities and accuracies of the said three methods were calculated and the contrastive analysis was made.Results For the hypertrophies in left or right atrium or ventricle or doule atriums or ventricles, the ECG plus VCG method had a high sensibility and accuracy.For the hypertrophies in right atrium and double atriums,the only ECG method had the same sensibility with the ECG plus VCG method.For the righ ventricle, the only VCG method had the same sensibility with the ECG plus VCG method.For the specificity, the three methods almost had the same value.ECG plus VCG method combining the clinical material could improve the acuracy in the diagnosis of atrial and ventricular hypertrophies.Conclusion Diagnosing the atrial or ventricular hypertrophy through ECG plus VCG will cover the shortage of ECG in such field. 　　Key Words：ECG；VCG；Diagnosis；Atrial hypertrophy；Ventricular hypertrophy 　　通过ECG十二导联P波、QRS波群以及VCG三个面P环、QRS环的异常改变分别诊断心房、心室肥大，比较ECG、VCG单独应用、ECG和VCG联合应用对心房、心室肥大诊断的临床符合率、阳性率、假阳性率，了解其敏感性、特异性、准确性的差异。现报道如下。 　　1资料与方法 　　1.1一般资料 随机选择2004年5月至2005年5月在我院进行健康体检的人员、门诊及住院病人，年龄在22岁~92岁，全部经过心脏听、叩
