精品论文 参考文献 保护性肺通气治疗重症胸部创伤并ARDS的疗效观察 陈强 DOI：10.7504/nk2016010203 中图分类号：R587.1 文献标识码：A 　　摘要：目的：保护性肺通气治疗重症胸部创伤并ARDS的疗效观察。方法：在我院治疗的患者中选取2014年6月—2015年9月收治并已确诊的重症胸外伤并ARDS患者50例，根据所有患者入院的编号进行平均分组，其中单号为对照组，双号为实验组，两组各为25例；实验组给予保护性肺通气治疗，对照组实施常规性的通气治疗方法，观察通气治疗24h后动脉血气并分析PaO2、PaCO2和氧合指数（PaO2/FiO2）进行通气前进行对比；同时对两组呼吸机辅助呼吸的时间、治疗效果以及死亡例数进行观察。结果：两组患者治疗前PaO2、PaCO2和氧合指数（PaO2/FiO2）,p值gt;0.05，不具有统计学意义；通气治疗后的第24h实验组的PaO2、PaCO2和氧合指数（PaO2/FiO2）明显高于对照组，且Plt;0.05；实验组的机械辅助呼吸平均时间为4.2plusmn;1.7（d）明显低于对照组的7.2plusmn;1.5（d）, Plt;0.05；对照组患者治疗的总有效率为64%，实验组为92%；实验组患者治疗的效果明显优于对照组。结论：保护性肺通气治疗重症胸部创伤并ARDS的治疗效果，能够提高患者的生活质量，是一种安全有效的治疗方法，值得在临床上普遍地推广。 　　关键词：重症胸部创伤；ARDS；保护性肺通气 　　Protective lung ventilation treatment of severe chest trauma and curative effect observation of ARDS 　　Chen Jiang Liu Juanli Sun Bin 　　(Qinghai province peoples hospita Intensive medicine 810001) 　　【abstract】objective：Protective lung ventilation treatment of severe chest trauma and curative effect observation of ARDS.methods:Patients treated in our hospital from June 2014 - September 2015 and has been confirmed and severe chest trauma patients with ARDS 50 cases, according to the serial number of all patients admitted to hospital for an average packet, including single number as the control group, double number as the experimental group, two groups of 25 cases.Protective lung ventilation was given controls implemented conventional ventilation treatment, observation of ventilation and arterial blood gas analysis after 24 h of treatment PaO2, PaCO2 and oxygenation index (PaO2 / FiO2) ventilation before comparing; The time of two groups of auxiliary breathing breathing machine, the effect of treatment and death cases were observed.results:Two groups of patients before treatment PaO2, PaCO2 and oxygenation index (PaO2 / FiO2), pvaluesgt; 0.05, no statistically significant; 24 h after the ventilation therapy group of PaO2, PaCO2 and oxygenation index (PaO2 / FiO2) is significantly higher than the control group, and P lt; 0.05,E
