精品论文 参考文献 二次胆道手术治疗肝胆管结石的临床分析 武汉市黄陂区人民医院外科 湖北武汉 430300 【摘 要】目的：分析二次胆道手术治疗肝胆管结石的临床疗效。方法：对我院在2013年02月到2015年01月收治的90例肝胆管结石患者进行手术治疗，90例患者中都有残留结石存在，其中22例患者是首次治疗手术，其余均进行二次胆道手术治疗。结石残留在肝内胆管的患者占38例，在胆总管的患者占26例，在肝内外胆道的患者占26例，分析分析二次胆道手术治疗肝胆管结石的临床疗效。结果：90例肝胆管结石患者进行二次胆道手术治疗后，存活的患者有88例，2例患者因为出血过多而死亡，死亡率在2.2%左右。术后出现2例患者胸腔积液、2例患者出现了膈下感染现象，4例患者出现了肝脓肿，6例患者发生了胆道出血，2例患者出现了肠瘘，术后的发生的总并发症率是17.8%。经过及时的发现和合理正确的处理???，88例患者都在治愈后出院。结论：通过对我院的肝胆管结石患者进行二次胆道手术治疗的临床疗效观察，二次胆道手术后的并发症发生率较高，因此对于第一次手术治疗后出现的结石残留和结石复发的患者要给以重视，确诊后进行积极地治疗，注重术后的护理治疗，减少肝胆管结石的复发率，提高首次手术的成功率。 【关键词】二次胆道手术；肝胆管结石；临床分析 [Abstract] Objective：to analyze the clinical effect of the treatment of two biliary tract surgery in the treatment of bile duct stones. Methods：in our hospital in February 2013 to 2015 January were 90 cases of hepatolithiasis patients were treated surgically，90 patients with residual stones exist，including 22 cases of patients is the first surgery，the rest were secondary biliary surgery. Residual stones in the intrahepatic bile duct in 38 cases，26 cases of patients in the common bile duct，26 cases of patients with biliary tract in liver and outside，analysis and analysis of the clinical efficacy of two biliary duct surgery in the treatment of bile duct stones. Results：90 cases of patients with bile duct stones after the two biliary surgery treatment，the survival of patients with 88 cases，2 patients because of excessive bleeding and death，the mortality rate of about 2.2%. Postoperative appeared in 2 cases of patients with pleural effusion and 2 cases appeared subphrenic infection，4 cases with liver abscess，6 patients occurred biliary tract hemorrhage，2 cases of patients with intestinal fistula，postoperative occurrence of the total complication rate was 17.8%. After a timely discovery and reasonable treatment，88 patients were discharged from hospital after treatment.
