精品论文 参考文献 急性闭角型青光眼合并轻度白内障两种手术方式比较 河南罗山县人民医院眼科 464200 摘要：目的：探讨两种手术方式治疗青光眼合并轻度白内障的临床疗效。方法：选取我院原发性闭角型青光眼合并轻度白内障患者78 例（85 眼），分为对照组和实验组，对照组39例（42眼），实验组39例（43）眼，对照组采用两阶段手术（先施行小梁切除术，待白内障发展到一定程度再行超声乳化白内障吸除人工晶状体植入）治疗。实验组采用联合手术（超声乳化白内障吸除人工晶状体植入联合小梁切除术）治疗。比较两组患者的疗效及并发症情况。结果：术后所有患者视力均有不同程度提高，实验组优于对照组；两组术后平均眼压较术前均明显下降（P lt;0.01），两组间术后平均眼压比较，差异无显著性（Pgt;0.05），实验组并发症发生率明显低于对照组，差异具有显著性。结论：与两??段手术相比，超声乳化白内障吸除人工晶状体植入联合小梁切除术治疗闭角型青光眼合并轻度白内障，具有视力预后好、并发症发生率低等优点。 关键词：青光眼；轻度白内障；两阶段手术；联合手术 Comparison of two methods for glaucoma combined with a cataract [Abstract] Objective：To investigate the clinical efficacy of the two methods to treat tahe ngle-closure glaucoma with cataract.Methods：A total of 78 cases（85eyes）with glaucoma and coexisting cataracts were selected and assigned into two groups，the experimental group，39 cases（43 eyes）and the control group，39cases（42eyes）.The control group was treated with staged procedure，after successful trabeculectomy，underwent cataract surgery by phacoemul-sification techniques and intraocular lens implantation.The experimental group was treated with triple procedure，cataract phacoemulsification and intraocular lens implantation combined with trabeculectomy with peripheral.Results：All patientsrsquo; visual acuity were improved，The effects of experimental group were better and the differences between the two groups had remarkable statistics meaning.（P＜0.05.The average postoperative intraocular pressure of the two groups were significantly decreased compared to preoperative，and there was no significant difference between two groups.And the incidence of complications in experimental was significantly lower than control group.Conclusion：Compared with the staged procedure，combined phacoemulsification，intraocular lens implantation and trabeculectomy has good vision prognosis and lower incidence of complications for angle-closer glaucomatous patients with coexisting cataract and better visual acuity.
