老年慢性病患者反复入院体验的质性研究

[ 摘 要 ] 目的:了解老年慢性病患者的反复入院体验。方法:采用描述性质性研究方法,对14 名有反复入院经历的老年
慢性病患者进行访谈,对资料进行内容分析。结果:受访者认为疾病难以控制和社区卫生资源不足是导致反复入院的主要原因,
感受到疾病管理、躯体不适、活动自理受限、死亡交流困难、经济和照顾的压力,需要医院、家庭和社会的支持;患者并不奢
望治愈疾病,希望控制症状,减少痛苦和住院次数。结论: 了解老年慢性病患者对反复入院的原因认知、压力及支持力量的感知、
住院期望等体验有助于指导医务人员完善延续性护理,以减少再入院及改善患者的住院体验。

[Abstract] Objective: To understand the experiences of repeated hospitalization among elderly patients with chronic
diseases. Method: Descriptive qualitative method was used. 14 elderly patients with chronic diseases and repeated hospitalization
were interviewed, and content analysis was used for data analysis. Results: The participants identified uncontrollable disease
process and insufficient community healthcare resources as the main reasons for repeated hospitalization. They experienced
stresses from disease management, discomfort, impaired self-care, financial and care burdens, and difficulty in discussion about
death issues. They believed supports from hospital, family and social network were very important. They no longer expected
the diseases to be cured during the hospitalization. They preferred more efforts on symptom control, reduction of sufferings
and re-hospitalization. Conclusions: Understanding elderly patients perceptions of repeated hospitalization, stresses, supports,
as well as their expectations, will facilitate health care providers efforts on ensuring continuity of care, so as to reduce repeated
hospitalization among elde
