精品论文 参考文献 无痛胃镜联合肠镜检查并发症原因分析及人性化护理的应用效果 王连桂 杨莹 　　[摘 要]目的：分析无痛胃镜联合肠镜检查并发症原因，评价人性化护理应用效果，总结护理经验。方法：筛选无痛胃镜联合肠镜检查患者纳入研究，常规护理患者86例纳入常规组，给予人性化护理患者98例纳入人性组，对比不良事件发生情况、相关指标变化情况。结果：人性组，麻醉前、患者苏醒后5minMPA、HR、RR水平低于同期常规组，差异具有统计学意义（P＜0.05）；人性组不良事件例次率低于常规组，差异具有统计学意义（P＜0.05）；合计，低氧血症发生率4.89%、心率波动11.41%、血压波动5.43%、心律失常2.17%、胃穿孔1.09%、胃出血0.54%，并发例次率26.09%。结论：无痛胃镜联合肠镜检查并发症原因较复杂，与护理质量有关，还与患者基础疾病、术前应激水平、麻醉水平、术中操作等因素有关，人性化护理有助于改善患者应激水平，预防心率、血压波动，降低循环系统相关并发症发生风险，但对于呼吸系统并发症预防效果并不明显。 　　[关键词]无痛胃肠镜 并发症 人性化护理 　　DOI:10.3760/cma.j.issn.1672-7088.2015.27.24 　　作者单位：250022，山东济南，济南市第五人民医院 　　作者简介：王连桂（1966—），女，本科，济南市第五人民医院主管护师。 　　Painless Gastroscope Combined Endoscopic Check Cause Analysis and Humanized Nursing Application Effect 　　Wang Liangui,Yang Ying 　　Abstract:Objective: to analyze painless gastroscope combined endoscopic check reason, evaluate humanized nursing application effect and sum up nursing experience.Methods: To select 86 painless gastroscope combined endoscopic check patients as routine group, 98 patients as humanity group, and compare their adverse events and related indicators. Results: humanity group with anesthesia, patients after waking up before 5 min MPA, HR and RR level was lower than routine group, it featured statistically significantly difference (Plt;0.05); Human nature of adverse events cases rate was lower than routine group, it had statistical significance (Plt;0.05); In all, 4.89% incidence of hypoxemia, heart rate fluctuations 11.41%, blood pressure, arrhythmia, 11.41% and 5.43% 1.09%, 1.09% for gastric perforation, stomach bleeding, concurrent cases rate 26.09%. Conclusion: Painless gastroscope combined with endoscopic check cause is complex, related with nursing quality, basic diseases, patients with preoperative stress level, level of anesthesia, intraoperative factors, it is helpful to improve patientsrsquo; stress level, heart rate,blood pressure fluctuations, circulation system, however, it is not obvious forrespiratory complications prevention effect
