AbstractObjective:ToevaluatetheclinicvalueofDW-MRIintheearlydiagnosisofresidual lesionsornewlesionsandthequantitativeevaluationofDWIforthelesionsafter RFA of hepatic carcinoma.Materialsandmethods:ThedataofDW-MRIof31patientswithhepaticcarcinoma beforeandafterRFAandof20normalpersonswereanalyzed.ThelesionsafterRFA intheDWIandthechangeofsignalofROIwereanalyzedintheDWIandADC imaging,andADCvaluesoflesionsweremeasuredbeforeRFAanddifferent organizationsafterRFA.ThelimitingvalueofADCwasobtainedwithpathologyor DSA imagesin control.Results:Thesensitivity,specificity,PPVandNPVofCTimagesin28casesand enhancedMRIin31casesbeforeRFAwereanalyed,whichshowedthehighestsensitivity (100%) ofcontrast-enhanced MRI（CE-MRI）combined withDWI. Resultsshowedthatthesignaloflesionswasmoreaccuratewhenbvaluewas600and 800s/mm2 inDWI.Thirtyonepatientswith38lesionsbeforeandafterRFAwere analyzedintheroutineMRI,CE-MRIandDWI,andADCvaluesofimagewere measured.ADCvaluesoflesionswereacquiredbyusingofoveralllevelmeasurement andhighandlowsignalareameasurement,thedifferencesofADCvaluesbetween lesionsandnormalliverintheADCimagingwithbvalue800s/mm2weresignificant (P0.05).DifferencesofADCvaluesbetweenlesionsandlivertissuearoundlesions, the lesions and inflammation tissue, the lesions before RFA and afters, were significantintheADCimagingwithbvalue600and800s/mm2(P0.05),wherethe differences of ADC values were no statisticallysignificantintheADCimagingwithb value 400 s/mm2 (P0.05). Differences of ADC values between lesions and coagulation necrotic tissue, lesions and liquefaction necrosis after RFA were significantin the ADC imaging with b value 400, 600 and 800 s/mm2(P0.05). Conclusion:DWIdisplaysahighersensitivityfortumordetection.AndthedetectioncapacitycouldbeevenimprovedbycombinationofDWIandMRI.ADCvalue measuredisneartothetrueDCvalueintheADCimagingwhenbvaluewas800 s/mm2,ifthelesionshowedaclearboundary.DWIcombinedwithADCimaging couldshowsignaldifferencesbetweentumorsandothertissues,suchasnormalliver parenchyma,inf
