H型高血压与颈动脉结构及血流动力学变化相关性研究 　　[摘要] 目的 探讨H型高血压与颈动脉结构及血流动力学变化的相关性。 方法 将2013年8月～2015年8月广东省佛山市顺德区第一人民医院附属杏坛医院（以下简称“我院”）收治的75例H型高血压患者和64例单纯性高血压患者分别作为A、B组，同时将同时期来我院体检的73名健康志愿者作为C组，所有研究对象行常规检查、血生化检查及颈动脉超声检查，比较三组之间的检查结果，并采用Spearman等级相关分析同型半胱氨酸（Hcy）水平与颈动脉结构及血流动力学指标的相关性。 结果 三组研究对象Hcy水平比较差异有统计学意义（P 0.05）。三组研究对象颈动脉血管内径、内膜-中层厚度（IMT）、舒张末期血流速度（EDV）、血管阻力指数（RI）、血管搏动指数（PI）、收缩气峰值血流速度（PSV）、平均流速（Vean）、最大剪切率（SR-max）比较，差异有统计学意义（P 0.05），且两两比较差异均有统计学意义（P 0.05）。H型高血压患者Hcy水平与IMT、RI呈正向相关（r=0.365、0.351，均P 0.05），与SR-max呈负向相关（r=-0.412，P 0.05）。 结论 H型高血压患者的颈动脉结构及血流动力学指标异常，推测和血浆Hcy水平异常升高存在紧密关联。 　　[关键词] H型高血压；颈动脉结构；血流动力学变化；颈动脉超声；同型半胱氨酸 　　[中图分类号] R544.1 [文献标识码] A [文章编号] 1673-7210（2016）07（b）-0107-04 　　[Abstract] Objective To discuss the relationship between the type H hypertension and the structure of carotid artery and the changes of blood flow. Methods 75 cases of patients with type H hypertension and 64 cases of patients with simple hypertension treated in Xingtan Hospital Affiliated to the First Peoples Hospital of Shunde District in Foshan City （“our hospital” for short） from August 2013 to August 2015 were respectively taken as the group A and group B， and another 73 cases of healthy volunteers taken physical examination in our hospital during the same period were selected as the group C.
