凝血指标测定对诊断胎盘早剥价值分析 　　摘 要：目的 探?血清D-二聚体、纤维蛋白原测定对胎盘早剥的诊断价值。方法 选取本院2015年1月～2017年4月确诊为胎盘早剥产妇36例为观察组，选取正常产妇40例为对照组，分别测定胎盘早剥产妇、正常产妇术前血清D-二聚体、纤维蛋白原、部分凝血活酶时间，比较观察组与对照组以上指标是否有差异。胎盘早剥组产妇术前依据B超诊断24例，B超与凝血指标联合诊断32例，比较两种诊断方法是否有差异。结果 胎盘早剥产妇术前血清D-二聚体水平高于正常产妇（P0.05）。凝血指标联合B超诊断胎盘早剥确诊率高于单一B超诊断，差异具有统计学意义（P0.05）。结论 胎盘早剥前期血清D-二聚体水平升高，纤维蛋白原水平下降，上述两个指标可作为胎盘早剥诊断的依据，联合B超诊断可提高胎盘早剥的诊断率。 　　关键词：胎盘早剥；D-二聚体；纤维蛋白原；部分出凝血时间 　　中图分类号：R714.22 文献标识码：A DOI：10.3969/j.issn.1006-1959.2018.01.060 　　文章编号：1006-1959（2018）01-0153-03 　　Value Analysis of Coagulation Index in Diagnosing Placental Abruption 　　HE Zhen-zhen，WU Yan-hong，LIU Yan，ZHANG Ming 　　（Department of Obstetrics and Gynecology，Tianjin First Central Hospital，Tianjin 300192，China） 　　Abstract：Objective To investigate the serum D-two dimer，fibrinogen determination in the diagnosis of placental abruption.Methods In our hospital from January 2015～April 2017 confirmed the diagnosis of placental abruption mothers of 36 cases of the observation group，selected 40 cases of normal pregnancy as the control group，the determination of placental abruption mothers were normal pregnant women，preoperative serum D-two dimer，fibrinogen，activated partial thromboplastin time，the observation group compared with the control group the above index whether there are differences.The maternal placental abruption group based on preoperative B ultrasound diagnosis of 24 cases，ultrasound and blood coagulation index combined diagnosis in 32 cases，comparison of two diagnostic methods in diagnosing whether there are differences in placental abruption mothers.Results Placental abruption mothers preoperative serum D-two dimer levels higher than normal pregnant women（P0.05）.Blood coagulation index combined with ultrasound diagnosis of placental abruption diagnosis rate is higher than that of a single ultrasound diagnosis，the difference was statistically significant（P0.05）.Conclusion The level of serum D-two polymer was elevated in preplacental prophase.The level of fibrinogen is
