北京市右安门社区对糖尿病患者实施家庭医生式服务效果研究 　　[摘要] 目的 探讨北京市右安门社区对糖尿病患者实施家庭医生式服务的成效，为社区规范化糖尿病管理提供依据。方法 选取2013年7月～2014年6月与北京市丰台区右安门社区卫生服务中心签约的糖尿病患者4665例，以“3+X”的社区卫生服务团队为基础，开展家庭医生式服务模式对糖尿病患者进行规范化管理，12个月后评价成效。 结果 实施家庭医生式服务后，糖尿病患者糖尿病知识知晓率、规范管理率由实施前的56.91%、52.35%上升至82.68%、79.31%；糖化血红蛋白达标率、空腹血糖达标率由实施前的48.31%、34.84%上升至57.67%、43.56%，药费由实施前的（780±190）元降为（620±110）元；糖尿病患者对医疗机构及医务人员的满意度由实施前的76.91%、62.35%上升至91.68%、85.31%；差异均有统计学意义（P 0.05或P 0.01）。 结论 实施家庭医生式服务管理糖尿病患者治疗效果好、费用低、患者满意度高，“3+X”的社区卫生服务团队是其最好的实现载体。 　　[关键词] 糖尿病患者；社区卫生服务团队；家庭医生式服务；规范化管理 　　[中图分类号] R587.1 [文献标识码] A [文章编号] 1673-7210（2015）07（c）-0165-04 　　[Abstract] Objective To explore the effectiveness of family physician service in diabetic patients in Youanmen Community of Beijing， in order to provide the basis for standardized management of community. Methods 4665 patients with diabetes， who had signed with Youanmen Community Health Service Center of Beijing from July 2013 to June 2014， were selected to be carried out the family physician service pattern for standardized management of patients， based on 3+X community health service team. The results were evaluated after 12 months. Results The awareness rate of knowledge about diabetes mellitus and standardized management rate of diabetic patients underwent family physician services increased from 56.91% and 52.35% to 82.68% and 79.31%. The qualified rate of glycosylated hemoglobin and blood glucose increased from 48.31% and 34.84% to 57.67% and 43.56%. The costs of diabetic patients reduced from （780±190） yuan to （620±110） yuan. The degrees of satisfaction with institutions and medical personnel in diabetic patients increased from 76.91% and 62.35% to 91.68% and 85.31%. The differences of all above indexes were statistically significant （P 0.05 or P 0.01）. Conclusion Carrying out the family physician service to manage diabetic patients has good therapeutic effects， low costs and high patient satisfaction. The 3+X community health service team is the best implementing carrier of it.
