山西 山西医科大学硕士学位论文 I I β防御素-2 在天疱疮皮损中表达的研究 摘 要 目的： 研究 HBD-2 在天疱疮皮损中的表达部位及情况，进而发现其 HBD-2 与天疱疮 发病是否存在一定的联系。 方法： 取 20 例天疱疮患者皮损，20 例天疱疮患者中，男 10 例，女 10 例；寻常型银 屑病患者皮损共 20 例作为阳性对照组，其中男 7 例，女 13 例；取 20 例正常双眼皮 手术患者皮损作为正常对照组，其中男 6 例，女 14 例。 结果： HBD-2 在天疱疮皮损中的表达见于表皮基底层、棘层、颗粒层可以大量表达。 寻常型银屑病主要在基底层、棘层和颗粒层细胞的胞浆中表达。而正常皮肤基底层表 达 呈 阴 性 或 者 不 表 达 。 HBD-2 在 天 疱 疮 皮 损 中 的 表 达 较 正 常 皮 肤 及 银 屑 病 （Z=-11.188，P0.05；Z=-4.050，P0.05）均高。 结论： HBD-2 参与了天疱疮的发病过程，不仅能介导机体释放免疫因子，从而进行免 疫应答，并且一定程度上减少感染的发生，但由于防御素表达在其表皮全层，而天疱 疮患者表皮易破损脱落，因此在临床治疗上依然根据患者病情依然给予一定抗感染治 疗。 关键词：天疱疮；防御素-2；银屑病 II II Study of β-defensin-2 in the expressed of pemphigus Abstract Objective: By studying the HBD - 2 expression in pemphigus, HBD - 2 and pemphigus relationship of the disease. Mainly in the basal layer, the stratum spinosum with psoriasis vulgaris and granular layer cells, expressed in the cytoplasm of the hair follicles, sebaceous glands, sweat glands, and blood vessels are visible expression, and normal skin base layer were negative or weakly positive expression.
