早期视频脑电图检查在急性脑干卒中患者应用 　　摘 要 目的：探讨视频脑电图（VEEG）监测在神经重症监护室（NICU）脑干卒中患者中的早期应用价值。方法：对2010年1月-2014年6月NICU中的21例急性起病的脑干卒中患者进行VEEG检查，对结果进行分级，并分析VEEG结果与病情和预后的关系。结果：21例患者中脑干梗死13例，脑干出血8例。病灶位于桥脑+中脑3例，延髓+桥脑1例，延髓1例，桥脑16例。VEEG检测正常5例，轻度异常1 1例，中度异常2例，重度异常3例，异常率为76.2%。V E E G表现为中、重度异常的患者均预后不良；明确非惊厥性癫痫2例，均预后不良。结论：急性脑干卒中患者早期VEEG检查能提供脑功能补充评估，其结果为中、重度异常或合并非惊厥性癫痫者高度提示预后不良。 　　关键词 脑卒中 脑电图 预后 　　中图分类号：R743.3/R741.044 文献标识码：A 文章编号：1006-1533（2015）24-0010-03 　　Application of early video electroencephalographic examination in patients with acute brainstem stroke 　　WANG Qinying1， YU Min 1， GU Qin1， JIANG Dudu1， WANG Xing2， ZHAO Lijuan2， DENG Bing2， LU Jingjue1， ZHAN Qing1（1.Depentment of Neurological Rehabilitation， the Seventh People’s Hospital， Shanghai 200137， China； 2.Depentment of Neurological 　　Intensive Care Unit，Tongji Hospital affiliated to Tongji University， Shanghai 200065， China） 　　ABSTRACT Objective： To investigate the value of early vedio electroencephalographic （VEEG） examination in patients with acute brainstem stroke admitted to the neurological intensive care unit （NICU）. Methods： VEEG examination was performed in 21 patients with acute brainstem stroke in the NICU from Jan. 2010 to June 2014. The findings of VEEG examinations were classified， and the relationship between outcomes and the findings was analyzed. Results： In 21 patients， infarction was found in 13 patients， and hemorrhage in 8 patients. The lesion was found in the pons and mesencephalon for 3 patients， in medulla and pons for 1 patient， in medulla for 1 patient， and in pons for 16 patients. The VEEG findings showed that 5 cases were normal， 11 cases were mild abnormal， 2 cases were moderate abnormal and 3 cases were severe abnormal. Non-convulsive seizures （NCS） were identified in 2 cases. It was indicated that the patients with moderate or severe abnormal findings of VEEG or NCS had poor outcomes.
