腋入路腔镜甲状腺手术中电凝钩应用体会 　　[摘要] 目的 探讨电凝钩在腔镜甲状腺手术中全程应用的可行性。 方法 回顾性分析2014年1月～2015年3月共43例进行腋窝入路腔镜甲状腺手术患者的临床资料，其中19例术中应用电凝钩（电凝钩组），24例术中应用超声刀（超声刀组）。对比分析两组患者的治疗情况。 结果 电凝钩组与超声刀组在年龄、肿瘤大小、肿瘤性质、手术时间、引流量、住院时间、并发症及不良反应等方面差异无统计学意义（P0.05）。两组住院、手术费用差异有统计学意义（P0.05）。 结论 腔镜下电凝钩的全程使用在腔镜甲状腺手术中是安全可靠的，可以达到与超声刀相近的手术效果，并不增加并发症的发生率，节约手术成本，降低患者的费用，有良好的社会效益。 　　[关键词] 电凝钩（单极）；超声刀精细解剖血管移行凝闭；经腋入路腔镜甲状腺手术 　　[中图分类号] R653 [文献标识码] B [文章编号] 1673-9701（2016）13-0051-04 　　Application experience of electrocoagulation hook in the operation of endoscopic thyroidectomy via axillary approach 　　ZHANG Weixing DENG Weiping LI Ming YUAN Lu 　　Department of General Surgery， Futian Peoples Hospital of Shenzhen City， Shenzhen 518033， China 　　[Abstract] Objective To explore the feasibility of application of electrocoagulation hook in the whole course of endoscopic thyroidectomy. Methods Clinical data of 43 patients who were given endoscopic thyroidectomy via axillary approach from January 2014 to March 2015 were retrospectively analyzed. Electrocoagulation hooks were applied in 19 cases （electrocoagulation hook group） while ultrasound knives （ultrasound knife group） were adopted in 24 cases during the operation. The treatment efficacy of patients from both groups were compared and analyzed. Results No significant differences were presented regarding age， tumor size， tumor nature， operation duration， drainage volume， hospitalization duration， complications and adverse events of the patients from both groups（P0.05）. There were significant differences in the two groups in hospital and operation cost （P0.05） Conclusion The application of electrocoagulation hook in the whole course of endoscopic thyroidectomy is safe and reliable with the similar operation effect of ultrasound knife without increasing the occurrence rate of complications， which can save the operation cost and decrease the expense of the patients with good social benefits. 　　[Key words] Electrocoagulation hook （single pole）； Ultrasound knife fine dissectio
