解剖型锁定钢板治疗42例肱骨近端骨折临床效果分析 　　[摘要] 目的 评价解剖型锁定钢板治疗肱骨近端骨折的临床疗效，分析其优缺点。 方法 选取本院骨科2011年1月～2015年12月收治的42例肱骨近端骨折患者，按Neer分型II型骨折15例，III型骨折19例，IV型骨折8例，给予骨折切开复位、解剖型锁定钢板固定，术后定期复查X线片，了解骨折愈合及并发症发生情况。同时在末次复诊时按照Constant-Murley肩关节评分标准进行功能评分。 结果 术后随访12～36个月，平均18个月。37例患者达到骨折临床愈合，总有效率为88.1%，优良率为78.6%，术后主要并发症是肱骨头坏死（4例）、骨折畸形愈合（4例）和骨性关节炎（2例）。 结论 解剖型锁定钢板治疗肱骨近端骨折临床效果好，值得临床推广。 　　[关键词] 肱骨骨折；骨折固定术；锁定钢板 　　[中图分类号] R687.3 [文献标识码] A [文章编号] 2095-0616（2017）14-206-03 　　Analysis on clinical effect of anatomic locking plates in treatment of proximal humeral fractures in 42 cases 　　HUANG Zhonglian ZHENG Chuangyi 　　Department of Orthopedics，The First Affiliated Hospital of Shantou University Medical College，Shantou 515041，China 　　[Abstract] Objective To evaluate the clinical effect of anatomical locking plates in treatment of proximal humeral fractures to analyze advantages and disadvantages. Methods 42 patients with proximal humeral fractures who were admitted to department of orthopedics in our hospital were selected.According to Neer classification，there were 15 cases with type II fractures，19 cases with type III fractures and 8 cases with type IV fractures.They were given open reduction of fracture and fixation with anatomical locking plates.X-ray was regularly reviewed after surgery to know fracture healing and the occurrence of complications.In addition，Constant-Murley shoulder joint assessment standard was used to assess function at the last return visit. Results All patients were followed up from 12 to 36 months after surgery，and the average was 18 months.Fractures of 37 patients reached clinical healing.Total effective rate was 88.1% and the excellent and good rate was 88.1%.Postoperative complications were mainly humeral head necrosis （4 cases），malunion （4 cases） and osteoarthritis（2 cases）. Conclusion Anatomic locking plates in treatment of proximal humeral fractures has a good clinical effect，which is worthy of clinical promotion. 　　[Key words] Humeral fractures；Fracture fixation；Lock
