外耳道胆脂瘤治疗方法和预后关系 　　[摘要] 目的 观察与探讨外耳道胆脂瘤的治疗方法及与预后的关系。方法 Ⅰ期外耳道胆脂瘤在无麻醉的情况下取出胆脂瘤,氯霉素粉外耳道局部搁置24h;Ⅱ期手术治疗;并实施保健教育。对患者预后进行三年观察追踪。结果 Ⅰ期66耳无复发,扩大的外耳道有不同程度的好转,听力提高。用logistic回归分析,复发与病程和是否受到健康教育有关(P0.05)。结论 外耳道胆脂瘤的治疗方法可行,早期治疗、加强健康教育是预防疾病复发的重要因素。 　　[关键词] 外耳道胆脂瘤; 外耳道胆脂瘤治疗 　　[中图分类号] R739.61 [文献标识码] A [文章编号] 1673-9701(2010)05-158-02 　　 　　The Correlation of Treatment of External Auditory Canal Cholesteatoma to Prognosis 　　LIU Hao 　　Hospital of Xianjiaotong University,Xian 710049,China 　　 　　[Abstract]ObjectiveObservation and Discussion on the correlation of treatment of external auditory canal cholesteatoma to Prognosis.MethodsIn Phase I treatment of external auditory canal cholesteatoma,after the cholesteatoma has been removed without anesthesia,the external auditory canal is applied with chloramphenicol powder which remains for 24 hours. Phase II is the surgical treatment and health care information will be communicated to patients. A three-year prognosis observation and follow-up has been conducted with patients. ResultsAfter Phase I treatment,in 66cases there is no recurrence;there have been varying degrees of improvement in the expanded external auditory canals;and there has been improvement in the hearing ability of the patients.By logistic correlation analysis,recurrence has correlation with the course of disease and whether the patient has received relevant health care education(P0.05). ConclusionThe treatment method of external auditory canal cholesteatoma is feasible.Treatment at an early stage and strengthening health education are important factors in preventing disease from recurring. 　　[Key words]External auditory canal cholesteatoma; Treatment of external auditory canal cholesteatoma 　　 　　外耳道胆脂瘤是阻塞于外耳道含胆固醇结晶的脱落上皮团块所包裹的一种慢性炎症。原因不明,临床上并不少见,如不及时治疗可压迫外耳道骨组织,使外耳道扩大,重者侵犯中耳。本文对1995～2007年门诊收治的外耳道胆脂瘤患者69耳临床资料进行分析,现报道如下。 　　1 资料与方法 　　1.1 研究对象 　　我院1995～2007年门诊收治外耳道胆脂瘤共69耳,单耳67例,双耳2例。其中男性42例,43耳;女性26耳。年龄20～70岁间,平均年龄(46±43)岁,均无洁耳史。因耳痛、耳闷涨就诊36例。因听力下降、反复有耳部流脓史、耳鸣就诊的30例。其中
