中 文 摘 要 反复性自然流产（recurrent spontaneous abortion，RSA）是指自然流产连续发 生 3 次或以上者，过去常称习惯性流产。其西医学病因复杂，常见的有遗传学、 解剖学、内分泌、免疫、感染、环境因素等，但是约有 50%RSA 的原因不明。中 医学称此病为“滑胎”，又名“数堕胎”。中医认为其病因病机分为胎元和母体两 方面，与情志因素、淫邪因素、生活与体质因素有关进而引起肾、肝、脾的功能 失常，气血、冲任失调，现代中医妇科学结合西医学理论与实践对 RSA 进行了大 量的实验和临床研究，分别从辨证分型论治、辨病专方治疗、针灸治疗、针药联 合治疗、中西医结合治疗等方面进行治疗。西医着重于探讨免疫学因素所致 RSA 的免疫治疗。笔者通过查阅大量文献，拟将近 10 多年来中、西医对 RSA 的研究 进展作一论述，并对 75 例脾肾两虚型早期先兆流产做了临床小结分析，同时介绍 导师对反复性自然流产、先兆流产认识及其临床治疗经验。通过对现有文献资料 的分析，笔者还提出了当前 RSA 的研究中存在的问题和展望。 关键词： 反复性自然流产 中西医结合 研究进展 临床小结 Abstract Recurrent spontaneous abortion is refer to natural abortion occurrences for three times or above three times. It is used to calling as customal miscarriage. The cause of this disease in western medicine is complex, the most common reasons are Genetics、 Anatomy、internal secretion、immunity、infection and enviroment etc.But there is also 50% reason of RSA unknown. TCM call this sickness is “habitual abortion”,other name “number abortion”. TCM thought its cause of disease pathogenesis divides into the parent substance two aspects. With the sentiment will factor, the immoral factor, the life and the physique factor related then cause the kidney, the liver, the spleen function is abnormal, qi and blood ,chongren no matter what is out of balance. According to the knowledge and clinic practice of present gynecology of TCM and western medicine, many people take considerable empirical studies and clinical researches about RSA.Its therapeutic methods include syndrome or disease differentiation and treatment、special recipe of TCM、combined treatment of TCM and western medicine、acupuncture therapy. The western medicine emphatically is the result of to discussion immunology factor the RSA immunity treatment. The author summarizes the advancement of researches of TCM and western medicine about RSA in last 10 years through referring a lot of literatures.
