综合心理干预治疗首发老年抑郁症疗效评价 　　[摘要] 目的 评价综合心理干预治疗首发老年抑郁症的疗效。方法 方便选取2015年2月―2016年8月门诊体检筛查诊断为首发老年抑郁症患者为研究对象，收治入院184例，每收集2例患者，采用随机数字表达法分组，A组入选90例，B组94例。A组草酸艾司西酞普兰治疗，B组在用药基础上，运用综合心理干预治疗包括运动治疗、构建社会支持系统、工娱治疗、支持心理治疗、正念治疗，持续干预3个月。对比干预前后，汉密尔顿抑郁量表（HAMD）、老年人抑郁量表（GDS）评分情况。 结果 A组退出5例，B组退出4例。组内对比，3个月后A组、B组HAMD评分分别为（8.6±1.5）分、（7.1±1.8）分低于治疗前（11.1±1.3）分、（11.3±1.5）分，组间对比B组低于A组，差异有统计学意义（P0.05）。组内对比，A组、B组GDS评分分别为（12.9±2.5）分、（10.4±3.1）分，低于治疗前（18.4±2.6）分、（18.9±3.5）分，组间对比B组低于A组，差异有统计学意义（P0.05）。结论 抗抑郁药物治疗有助于减轻老年患者的抑郁水平，其中配合综合心理干预的效果更为显著。 　　[关键词] 老年人；首发抑郁症；综合心理干预 　　[中图分类号] R5 [文献标识码] A [文章编号] 1674-0742（2017）10（b）-0048-03 　　[Abstract] Objective This paper tries to evaluate the curative effect of comprehensive psychological intervention on the treatment of senile depression. Methods 184 patients diagnosed as first-episode senile depression by outpatient physical examination from February 2015 to August 2016 conveniently were enrolled in the study and randomly divided into two groups by random number method： group A（n=90） and group B（n=94）. Group A was treated with escitalopram oxalate. Group B adopted comprehensive psychological intervention on the basis of medication， including exercise therapy， social support system， utility treatment， supportive psychotherapy， orthodox treatment and continuous intervention for 3 months. The Hamilton Depression Scale （HAMD） and the Geriatric Depression Scale （GDS） score before and after intervention were compared. Results 5 patients were withdrawn from Group A， 4 patients were withdraw from group B. After three months， the HAMD score was （8.6±1.5）points and （7.1±1.8）points in group A and group B， lower than those before treatment of （11.1±1.3）points and （11.3±1.5）points respectively； and that of group B was lower than group A， the difference was statistically significant （P0.05）.
