镇痛泵用于产后镇痛的的疗效观察 　　【摘要】 目的 探讨镇痛泵配以曲马多注射液与枸橼酸舒芬太尼用于产后镇痛的疗效及副作用。方法 观察镇痛泵配以曲马多注射液与枸橼酸舒芬太尼用于106例产妇的镇痛效果,并以91例产妇采用传统镇痛方式作为疗效对照。结果 在产后4h、8h、12h及24h四个时间点上,观察组疼痛分值均明显小于对照组(P0.001)。观察组产后开始泌乳时间明显早于对照组(P=0.000),24h内母乳喂养次数亦明显多于对照组(P=0.000)。观察组大便干结、尿潴留的发生率高于对照组(P0.05),其他症状在两组间差异无统计学意义。结论 应用镇痛泵配以曲马多注射液与枸橼酸舒芬太尼用于产后镇痛,是一项疗效显著、副作用少且安全的技术。? 　　【关键词】 　　镇痛泵;产妇;曲马多注射液;枸橼酸舒芬太尼 　　?? 　　 　　【Abstract】 Objective To investigate the effect and side effect of analgesia with Tramadol Injection combined with Sufentanil Citrate for postpartum pain. Methods Observed the effect and side effect of analgesia with Tramadol Injection combined with Sufentanil Citrate in 106 cases of experimental group, made a comparison with 91 maternals with traditional pain control methods. Results The pain scores of experimental group was significantly lower than that of control group at postpartum 4 h, 8 h, 12 h and 24 h point(P0.001).The time of first lactation of experimental group was earler than control group（P=0.000），and the number of breastfeeding of experimental group was much more than control group during the first 24 hours（P=0.000）. The incidence of constipation and urinary retention of experimental group was significantly higher than that of control group（P0.05）,and other symptoms between the two groups showed no significant difference.Conclusion Analgesia with Tramadol Injection combined with Sufentanil Citrate for postpartum pain has significant effect, less side effects and safe technology.? 　　【Key words】 Analgesia; Maternal; Tramadol Injection; Sufentanil Citrate 　　? 　　产后疼痛对产妇是较大的影响,不利于哺乳及产后恢复。利用镇痛泵产后镇痛,疗效显著[1-2],2007年1月至2009年12月期间我院产科利用镇痛泵对106例产妇进行了术后镇痛,现对其疗效作一回顾性分析。? 　　1 资料与方法? 　　1.1 临床资料 本研究中观察组产妇106例,对照组产妇91例。年龄22~36岁,孕周37~43周,体质量64.5~71.2kg,乳房均发育良好。两组共有9例曾经行阑尾切除术。有31例曾经行人工流产,其中6例为药物流产。术前凝血全套检查均无明显异常。两组均为腹壁横切口,子宫下段剖宫产术,手术均顺利,无产科严重并发症。两组基线比较,年龄、孕周、体质量、既往腹部手术比例、既往流产比例、凝血全套各项指标、手术时间、术中出血量差异均无统计学意义(P0.05)。? 　　1.2 方法 观察组:产妇术毕保留硬膜外麻醉导管,连接浙江苏嘉医疗器械股份有限公司生产的一次性输注泵即镇痛泵。药物配制:枸橼酸舒芬太尼50ug加曲马多注射液400mg,
