高危型HPV筛查联合液基细胞学在宫颈癌筛检临床的应用分析 　　【摘要】目的：探讨薄层液基细胞学（TCT）与高危型人乳头瘤病毒（HPV）联合检测在宫颈癌及癌前病变筛查的临床意义。方法：以986例妇女为研究对象，分别行TCT、高危HPV及阴道镜病理学检查，以病理学检查为“金标准”对TCT、HPV及TCT+HPV联检进行方法学评价。结果：986例研究对象TCT检查ASC-US 31例、ASC-H 27例、LSIL 49例、HSIL 21例、SCC 9例；HPV检查，高危型HPV阳性201例，低危型HPV阳性19例；以病理学检查为“金标准”，TCT与HPV联检对灵敏度（86.9%）、约登指数（0.67）及阴性预测值（97.4%）均有不同程度提高。结论：TCT与高危型HPV的联合筛查，对宫颈癌的防治有重要的临床意义。 　　【关键词】人乳头瘤病毒；薄层液基细胞学；宫颈癌；宫颈上皮内瘤变 　　High-risk HPV screening combined liquid-based cytology in the screening of cervical cancerGUAN Guoqin1， WU Keping2， XU Bin3. 1. Methods： 986 cases of women were regarded as the research object， who were performed TCT， high-risk HPV and colposcopy pathological examination respectively. By using pathological examination as the “gold standard”， TCT， HPV and TCT + HPV joint inspection were methodologically evaluated. Results： Among the 986 cases of research objects having received TCT examination， there were 31 cases of ASC-US， 27 cases of ASC-H， 49 cases of LSIL， 21 cases of HSIL and 9 cases of SCC； in HPV examination， there were 201 cases of high-risk HPV-positive and 19 cases of low-risk HPV-positive； by using pathological examination as the “gold standard”， sensitivity （86.9%）， Youden index （0.67） and negative predictive value （97.4%） of TCT and HPV joint inspection were improved to a different degree. Conclusion： The joint screening of TCT and high-risk HPV is of important clinical significance for cervical cancer prevention and treatment. 　　【Key words】Human papillomavi
