小儿结核性脑膜炎临床诊治剖析 　　【摘要】目的 分析结核性脑膜炎的临床特点，为提高早期诊断率，积极治疗提供帮助。方法 对我院2004年1月～2010年2月收治的80例结核性脑膜炎患儿的临床资料进行回顾性分析。结果 结核性脑膜炎患儿临床症状不典型，特别是早期表现各异，常伴发热、头痛、呕吐，各项辅助检查阳性率不高，需综合分析，影像学检查部分患儿可见异常。结论 做到早期诊断和正确的综合治疗是决定小儿结核性脑膜炎治疗的关键。 　　关键词: 结核性脑膜炎 不良反应 诊疗 　　Pediatric tuberculous meningitis clinical diagnosis analysis 　　【Abstract】 Objective Analysis of the clinical features of tuberculous meningitis, in order to improve the early diagnosis, aggressive treatment to help.Method From 2004 to January in February 2010 drug-induced 80 cases of tuberculous meningitis patients clinical data were retrospectively analyzed Results Tuberculous meningitis patients typically clinical symptoms, especially in early, usually associated with different performance of fever, headache, vomiting, the rate of auxiliary examination is not high, need comprehensive analysis, imaging examination abnormal. Children visible part ConclusionDo early diagnosis and correct treatment of pediatric tuberculous meningitis is to determine the key treatment. 　　【Key words】Tuberculous meningitis ; Adverse reaction ;diagnosis 　　中图分类号：R529.3 文献标识码：B 文章编号：1004-7484（2011）06-0057-02 　　 　　结核性脑膜炎(TBM)是最严重的一种肺外结核, 简称结脑，是严重威胁小儿生命健康的神经系统感染性疾病。其病死率为15％ ～36％ ，存活者后遗症较多，小儿TBM临床表现特异性差，容易延误诊断甚至误诊，从而影响疗效及预后，早期、及时的诊断和治疗是降低结脑病死率和致残率的关键[1]。现回顾分析我院2004年1月～2010年2月临床诊断TBM的病例80例，分析其临床特点，以期对小儿TBM 的早期诊治提供帮助和指导，对临床医生的诊断治疗有所帮助。 　　1 材料与方法 　　1.1 一般资料：我院2004年1月～2010年2月收治的80例结核性脑膜炎患儿,男性52例，女性28例。年龄:5个月至13岁，其中5个月--1岁10例，1～4岁34例，5～1O岁26例，11～14岁10例。未接种卡介苗26例，接种卡介苗54例，有明确结核接触史18例。病程(发病至就诊时间)1---5天者20例，1―2周40例，2周～1个月20例。见表1 　　表1 80例患儿一般资料 　　资料 组别 例数（%） 　　性别 　　 　　 　　 　　年龄 　　 　　 　　病程 男 　　女 　　5个月--1岁 　　1～4岁， 　　5～10岁 　　11～14岁 　　1---5天 　　1―2周 　　2周～1个月 52（65） 　　28（35） 　　10(12.5) 　　34(42.5) 　　26(32.5) 　　10(12.5) 　　20（25） 　　40（50） 　　20（25） 　　1.2 临床表现： 　　发热者60例(75％)，头痛20例(25％)，呕吐30例(37．5％)，抽搐25例(31.25％)，嗜睡24例（30%），肢体活动障碍27例(33.75％)，意识障碍18例(22.5％)。 　　1.3 辅助检查： 　　80例行CT、强化CT检查，影像学有异常表现72例(90．O％)，头颅CT渗出性改变55例，脑室系统扩张、脑积水56例，脑膜结核瘤或脑室质内结核瘤3
