老年上消化道出血病因及相关因素对比剖析 　　【摘要】目的 探讨老年上消化道出血的病因，并就相关因素进行分析。方法 对同期因上消化道出血在我院住院的218 例老年患者及186 例非老年患者的临床资料进行统计分析。结果 老年组胃溃疡、胃癌致上消化道出血及不明原因出血的发病率高于非老年组。老年组的十二指肠球部溃疡发病率、幽门螺杆菌感染率低于非老年组；老年组肝硬化致上消化道出血发病率与非老年组相似。结论 老年上消化道出血中，胃溃疡、胃癌以及不明原因出血者高于非老年组。 　　【关键词】老年人 上消化道出血 病因 　　中图分类号：R573.2文献标识码：A文章编号：1005-0515（2011）10-031-02 　　Comparative analysis of the causes and the relevant factors of upper gastrointestinal bleeding in the elderly 　　YANG Hongbo 　　(Department of Gastroenterology,Luzhou People,s Hospital, 646000,China) 　　【Abstract】Objective To explore the causes of upper gastrointestinal bleeding in the elderly patients and to analyze the related factors in these patients. Methods To statistically analyze the clinical data of upper gastrointestinal bleeding in patients at a certain period, including 218 cases of elderly patients and 186 cases of non-elder patients. Results As compared with the patients in non-elder patients, higher incidence caused by gastric ulcer, gastric cancer induced upper gastrointestinal bleeding and the undetermined causes were observed in the elderly patients. The elderly had a lower incidence of duodenal ulcer and infection rate of helicobacter pylori than the non-elder group. There was no statistical significance in incidence caused by liver cirrhosis between the elder and non-elder patients. Conclusions As compared with non-elder patients, higher incidence caused by gastric ulcer, gastric cancer and the undetermined causes were observed in the elderly patients of upper gastrointestinal bleeding. 　　【Key words】the elderly upper gastrointestinal bleeding etiology 　　上消化道出血是消化道常见病，老年患者对上消化道出血的代偿适应能力差，常伴更多、更严重的合并症而死亡率高，预后差[1]。为探讨老年上消化道出血的病因，本文对2007 年1 月至2010 年12 月我院收治的404例上消化道出血患者，对照研究老年上消化道出血的病因及相关因素, 现报告如下： 　　1资料与方法 　　1.1入选标准入选标准为：有呕血或黑便症状, 经胃镜或上消化道钡餐检查证实有上消化道病变，未经上述检查者以呕血、柏油样黑便，引流物隐血阳性为临床诊断依据。年龄≥60 岁者为老年组，年龄在18～59 岁者为非老年组。 　　1.2方法 　　收集研究对象的临床资料: 性别、年龄、胃镜、幽门螺杆菌 (快速尿素酶法及病理检测) 结果, 分析老年上消化道出血的病因及相关因素。 　　1.3统计方法 　　数据分析采用SPSS 11.5 统计软件包处理，计量资料以平均值x±s
