胎儿出生缺陷产前筛查超声诊断110例剖析 　　doi：10.3969/j.issn.1007-614x.2014.15.85 　　摘 要 目的：探讨产前超声筛查在胎儿缺陷诊断中的应用价值。方法：2007年8月-2013年8月收治出生缺陷患儿110例，对其临床资料进行回顾性分析。结果：产前超声检查共检出103例，出生缺陷检出率93.64%（103/110）；胎儿畸形病例中，单发畸形70.87%（73/103），多发畸形29.13%（30/103），其中心血管畸形所占比例最大，其他按比例大小依次为中枢神经系统畸形、消化系统畸形、颜面部畸形、颈部畸形、泌尿系统畸形、胎儿水肿综合征和四肢畸形。结论：产前超声检查在胎儿出生缺陷的诊断中具有较高的应用价值，值得临床借鉴和推广。 　　关键词 出生缺陷 胎儿 超声 产前筛查 　　Analysis of prenatal screening ultrasound diagnosis of 110 cases of fetal birth defects 　　Chen Yunfeng 　　Ultrasound Department，the Maternal and Child Health Hospital of Xinmi City，Henan 452370 　　Abstract Objective：To explore the application value of prenatal screening ultrasound in the diagnosis of fetal birth defects.Methods：110 children with birth defects were selected from August 2007 to August 2013.The clinical data was retrospectively analyzed.Results：The prenatal ultrasonography detected 103 cases，the detection rate of birth defects was 93.64%（103/110）.In fetal malformation cases，the single deformity was 70.87%（73/103），the multiple malformations was 29.13%（30/103）.The cardiovascular malformation accounted the largest proportion.The other in proportion to size were central nervous system abnormalities，digestive system abnormalities，facial deformities，neck deformities，urinary system malformations，fetal edema syndrome and limb malformations.Conclusion：Prenatal ultrasonography in the diagnosis of fetal birth defects has a high application value.It is worthy of clinical reference and promotion. 　　Key words Birth defects；Fetus；Ultrasonography；Prenatal screening 　　出生缺陷是指在母体中就已产生的有别于正常胎儿的一系列异常状况，主要表现为生理功能缺陷、代谢缺陷和形态结构异常[1]。其中生理功能和代谢缺陷的胎儿出生后易出现聋哑、智力低下等生理机能异常情况，形态结构异常的胎儿出生后易出现兔唇、脊柱裂等先天畸形[2]。婴儿存在出生缺陷时应及早进行准确诊断和治疗，而婴儿出生缺陷的类型、具体特点以及监测手段直接关系着出生缺陷早期诊断的准确性。超声检查具有安全性高、图像清晰、无创和可靠性强的特点，目前已成为应用范围较广的影像学检查方法之一。本文对超声诊断应用于出生缺陷产前筛查的实际效果进行了探讨，现报告如下。 　　资料与方法 　　2007年8月-2013年8月收治出生缺陷婴儿110例，孕妇年龄20～42岁，平均年龄（31.9±2.2）岁。其中8例有家族遗传史，15例智力低下，12例有异常发育史。 　　方法：检查前遵医嘱做好各项准备工作。通过讲解让受检对象明白相应的注意事项、禁忌和检查的意义等，努力消除其紧张心理。超声（型号730pro）检查时孕周23
