肝左右叶联合切除治疗复杂原发性肝胆管结石疗效剖析 　　doi：10.3969/j.issn.1007-614x.2014.21.19 　　摘 要 目的：对肝左右叶联合切除治疗复杂原发性肝胆管结石的疗效进行探讨与分析。方法：2010年12月-2013年12月收治复杂原发性肝胆管结石患者86例，对其临床资料进行回顾性分析，依照入院前后时间随机给予其中43例肝左右叶联合切除治疗（甲组），另43例采用肝叶与肝段切除治疗（乙组），对两组的治疗效果进行观察与对比。结果：通过相应治疗，在治疗优良率上，甲组95.3%（41/43），乙组76.7%（33/43），两组差异具有统计学意义（P0.05）；在并发症发生率上，甲组4.65%（2/43），乙组30.2%，甲组显著少于乙组，差异具有统计学意义（P0.05）。结论：对于复杂原发性肝胆管结石患者来说，运用肝左右叶联合切除治疗可起到显著的疗效，能够降低并发症发生率和减少残留结石，使患者的生活质量得到提高，值得推广。 　　关键词 肝左右叶联合切除 复杂原发性肝胆管结石 治疗效果 　　Curative effect of left lobe of liver resection in the treatment of complicated primary bile duct stones of liver 　　Li Yongzhi 　　Tht Seconed General Surgery Department of the Peoples Hospital of Dali State，Yunnan 671000 　　Abstract Objective：To evaluate the effect of left lobe of liver resection in the treatment of complicated primary hepatic bile duct stones.Methods：86 cases with complicated primary hepatic bile duct stones were selected from December 2010 to December 2013.We retrospectively analyzed the clinical data of them.In accordance with the admission time， 43 cases were given the left lobe of liver resection（group A），and the other 43 cases with liver lobes and hepatic segment resection（group B）.We observed and compared the therapeutic effect of two groups.Results：Through the corresponding treatment，in the treatment of the excellent and good rate，95.3% in group A（41/43），76.7%（33/43） in group B.The difference was statistically significant（P0.05）.In the complication rate，the group A was 4.65%（2/43），and the group B was 30.2%.The difference was statistically significant（P0.05）.Conclusion：For the complicated primary hepatic bile duct stones，the left lobe of liver resection in treatment can play a significant effect.It can reduce the incidence of complications and reduce the residual stone，and improve the quality of life of patients，so it is worthy of promotion. 　　Key words Left lobe of liver resection；Complicated primary hepatic bile duct stones；Effect of treatment 　　在临床外科当中，复杂原发性肝胆管结石属于常见病与多发病，临床处理为
