脉冲染料激光治疗婴幼儿体表血管瘤150例疗效剖析 　　[摘要]目的:研究595nm可调脉宽染料激光治疗体表婴幼儿血管瘤的疗效和适应证。方法:150例婴幼儿体表血管瘤接受595nm脉冲染料激光治疗,采用参数:能量7～14J/cm2,光斑直径7mm,脉宽0.5～10ms,治疗间隔15～30天,随访6个月以上。随机选择130例患儿不做任何治疗,跟踪观察瘤体变化1年,作为对照。结果:激光治疗组133例(88.7%)瘤体获得75%以上的消退,治疗后仅10例(6.7%)出现水疱,另1例(0.6%)破溃后遗留瘢痕;对照组130例仅13例(10%)获得75%以上的瘤体消退,增大109例(83.8%),破溃后遗留瘢痕16例(12.5%)。结论:595nm脉冲染料激光治疗早期体表婴幼儿血管瘤,创伤小、疗效高,瘢痕形成等不良反应低,可达到理想的美容治疗效果,适宜在临床推广使用。 　　[关键词]激光;血管瘤;婴儿 　　[中图分类号]R732.2[文献标识码]A[文章编号]1008-6455(2010)02-0233-03 　　 　　Efficacy analysis of 595nm pulsed dye laser for the treatment of 150 infantile superficial hemangiomas 　　YAN Dan, HUANG Ze-chun, YU Han, LI Shuai-hua, ZHANG An-li, TAN Zhang-mei, PENG Li, LI Xia-xia 　　(Department of Plastic and Cosmetic Surgery, the First Peoples Hospital of Chenzhou,Chenzhou 423000, Hunan,China) 　　 　　Abstract: ObjectiveTo investigate the clinical effect and indication of 595nm pulsed dye laser for infantile hemangiomas. Methods150 infantile superficial hemangiomas were treated with 595nm pulsed dye laser, correlated parameters are as follows: energy density 7～14J/cm2, spot diameter 7mm, pulse width 0.5～10ms.The treatment interval was 15～30 days,these cases were followed up for more than 6 months. Random selected 130 cases were into control group,they were followed up for one year without other treatment. Therapeutic effect and complication were observed.Result More than 75% of response was achieved in 133 cases (88.7%) in laser treatment group, side effects included bubble in 10 cases (6.7%),1 case(0.6%) had scar formation after ulcer. Good response was achieved in only 13 cases (10%) in control group, tumor volume increased in 109 cases (83.8%),16 cases(12.5%) had scar formation.Conclusion595nm pulsed dye laser for the treatment of superficial hemangiomas is a good tool,it has the advantage of little trauma, high efficacy and fewer side effects.
