华中华中科技大学硕士学位论文 PAGE 2 PAGE 2 第二部分：超声造影定量分析在评价肝脏良恶性肿瘤中的应用目的：评价超声造影定量分析在诊断肝脏良恶性肿瘤中的应用价值。方法：对 2008 年 1 月-2010 年 11 月在同济医院接受超声造影的 112 例患者造影图像进行 TIC 曲线分析，记录其峰值强度变化（A）、TIC 斜率（β）、A·β等指标。比较良恶性病灶上述指标的差异，并对其差异进行统计学处理，并用 ROC 曲线分析上述指标，得出对于判断良恶性病灶的最合适的定量值。结果：A 值对于判断肝脏的良恶性病变无统计学意义，β、A·β及峰值强度与延迟期的强度的差值在判别良恶性病灶差别有统计学意义，β、A·β及峰值强度与延迟期的强度的差值分别为 1.49、28.91、3.92db 时区别良恶性肿瘤有鉴别意义。肝血管瘤与肝癌周边与中心动脉晚期增强强度差值对于判别两者有统计学意义，差值取 5.72db 时对区别两者有鉴别意义。结论：超声造影中定量指标对于判别良恶性病灶有价值，β、A·β及峰值强度与延迟期的强度的差值分别取 1.49、28.91、3.92db 时对于判别良恶性病灶有鉴别意义，肝血管瘤与肝癌周边与中心动脉晚期增强强度差值取 5.72db 时对区别两者有鉴别意义。关键词：肝脏；超声造影；定量分析 Diagnosis of hepatic tumors and the relationship of the mode of the contrast -enhanced ultrasound and the generation of the tumor vessel ZHANG Shiming HUANG Daozhong Department of Ultrasound , Tongji Hospital , Tongji Medical College, Huazhong University of Science and Technology ,Wuhan 430030,P,R.China ABSTRACT PART ONE : The relationship of the mode of the contrast-enhanced ultrasound and angiogenesis of the hepatic malignant tumor. Objective ： the relationship of The mode of Contrast-enhanced ultrasonography(CEUS) and the angiogensis of the hepatic malignant tumor. Method：View the mode of the contrast-enhanced ultrasound of the 103 cases hepatic malignant tumor and compare with the color Doppler , Analyse the display capability of the blood vessel of the malignant tumor of hepatic ,and evaluate the capability of detecting the arteriovenous fistula of the hepatic malignant tumor. Results：the blood grading of Color doppler ultrasound and the size of the tumor have statistical significance with the blooding grading of the CEUS.and the boundary of the tumor and the Differentiation degree have no statistical significance with the blooding grading of the CEUS. The contrast-enhanced ultrasound can detect the arteriovenous fistula of the malignant tumor of the hepatic.
