胃癌在影像学诊断中价值 　　【摘要】目的：评价(DC CT)在胃癌诊断的价值。方法：回顾分析20例胃癌，进行胃肠(DC)气钡双对比造影和CT增强扫描，并经胃镜和手术病理确诊。结果：肿瘤对胃壁浸润范围及早期胃癌的诊断、肿瘤的定性、粘膜显示，表面轮廓等，DC检查比CT有明显的优势。而对于有无远处转移，CT具有最大的优势。结论：DC对胃癌的定性、定位诊断具有重要的作用，但有一定的漏诊率。CT对远处转移及邻近器官浸润占优势，综合CT DC检查技术可大大提高胃癌的诊断率，价值较高。 　　【关键词】胃癌 胃肠道气钡双重造影 体层计算机 　　 　　Imaging Diagnostic and Evaluation in Gastric Canaer 　　WEI GangSun GuojunLu Zhiyu (Department of Radiology，Yu Lin Da Liu Ta Kuang QU Hospital，719315，Shanxi，Province，China) 　　【Abstract】Objective： To assess the gastrointestinal tract double contrast radiography(DC) and body section calculating scan(CT) in diagnosing gastric cancer. Methods： retrospective analysis 20 cases gastric cancer. to carry out gastrointestinal tract double contrast radiography (DC)and CT enhancement scanning. also confirmed by operation or gastroscopey。Results： In oncoma duplet gastric wall infiltration ambit and early gastric cancerof diacrisis qualitationo tumor membrana mucosa the display and surface facial contour and so on .gastrointestinal tract double contrast radiography(DC) than CT enhancement scanning to ascendance，With regard to tumor distant place to shift generous of naugh CT phore maximal predominance，Conclusion，niveau diagnosis and qualitation diagnosis possess considerable role in gastrointestinal tract double contrast radiography. only unvarying in rate of missed diagnosis，CT a pair distant place metabasis vicinity cum part soakage to gain advantage.Combining DC and CT car raise the diagnostic accuracy of gasric cancer . 　　【Key Words】gastric carcinoma gastrointestinal tract double contrast radiography(DC) x- ray computer 　　 　　胃癌是我国的主要恶性肿瘤之一，其死亡率占所有恶性肿瘤死亡率23.02%[1]，具有发病率高、死亡率高、预后较差等特点。故早期发现，早诊断、早期治疗是降低死亡率的关键。近年来虽然影像诊断进展较快，但胃肠双对比造影仍(DC)是胃癌影像学诊断中的主要检查方法，虽内镜检测粘膜细微病变上优于胃肠双对比造影(DC)，且较直观，并可在内镜下活检和治疗。但胃肠双对比造影检查不需要麻醉，较内镜方便安全，患者易于接受，在胃的整体解剖和功能活动显示方面有其长处，因此胃肠双对比造影检查在胃癌检查中仍有相当重要的作用[2]。本文的宗旨在于综合应用影像学DC、CT增强扫描检查为胃癌诊断提供客观有力的条件，我们收集经手术病理证实胃癌20例，就DC和CT检查应用价值做如下探讨。 　　 　　1材料与方法 　　 　　1.1一般资料：搜集2005~2006年胃癌住院病例20例，所有病理均行
