三种治疗方案对强直性脊柱炎证型干预的研究.doc

IgA、方案一对 IgG 具有统计学差异（P＜0.05）；方案三与方案一对 BASDAI 评分和 PGA 评分均具有统计学差异（P＜0.05）；方案三对指地距改善具有统计学差异 （P0.05）。在肾虚督空证中，方案一对炎性指标具有统计学差异（P＜0.05）；三种 方案对 BASDAI 评分、PGA 评分改善具有有统计学差异（P＜0.05），三种治疗方案对 体格检查具有统计学差异（P＜0.05）。在肝肾两虚证中，方案一对血沉具有统计学差 异（P＜0.05）；方案二对 Ig 有统计学差异（P＜0.05），三种方案对 IgA 具统计学差 异（P＜0.05）。 结论：方案一对肾虚督空证的炎性指标、体格检查（指地距、枕墙距、Schober、 胸廓扩张度）、对湿热痹阻证、寒湿痹阻证、瘀血痹阻证的 IgG、对 BASDAI 和 PGA 评 分效佳。 方案二对湿热痹阻证、寒湿痹阻证、瘀血痹阻证 IgM；肾虚督空证的体格检查（指 地距、枕墙距、Schober、胸廓扩张度）；湿热痹阻证、寒湿痹阻证、肝肾两虚证的 BASDAI 和 PGA 评分效佳。 方案三对湿热痹阻证的炎性指标，免疫球蛋白、指地距、枕墙距、Schober、胸 廓扩张度以及 Bath 评分和自我评价均明显改善。 关键词  强直性脊柱炎；中医证型；治疗方案；干预研究 Intervention research of the three therapeutic schemes on the ankylosing spondylitis syndromes Speciality: Integrated Traditional Chinese Medicine and Western Medicine Author: Jiakaijun Tutor: Professor Liu Ying ABSTRACT Object: The purpose of the paper is to observe the clinical efficacy of the different syndromes of three different treatment programs on ankylosing spondylitis , to analyze what kind of treatment programs is the best and to provide a reliable basis for the treatment of ankylosing spondylitis . Methods: This study reviews and observes 75 cases of AS patients who were devided into five types of syndromes in traditional Chinese medicine. Through the main observations which include finger-ground distance, the pillow wall distance, Schober test, thoracic activity,erythrocyte sedimentation rate, C-reactive protein,Ig,active and inactive period ,the BASDAI scores and PGA scores ,we can study the relationship with TCM syndromes. Three clinical treatment programs: 1.Voltaren + kidney Juanbi pill;2. Voltaren + sulfasalazine; 3.Voltaren + tumor necrosis factor receptor antibody fusion protein (etanercept).The patients of each type of TCM syndrome are taken in three different treatment programs.Observing all above of these indexes,we can evaluate these patients` conditions.
