慢性肺源性心脏病临床诊疗探讨 方秋芳韩明锋(安徽省阜阳市第二人民医院236015) 【摘要】目的:通过对66例慢性肺源性心脏病临床资料分析，以探讨慢性肺 源性心脏病的临床特点及治疗措施。方法:对2008年1月至2013年12月住院病 例的临床表现、实验室及辅助检查和治疗措施进行回顾性分析。结果:慢性肺源 性心脏病的病因以慢性阻塞性肺疾病最常见，多有长期大量吸烟史；临床表现以 发热、咳嗽、咳白粘痰、呼吸困难、肺部罗音、紫组和浮肿为主，咳痰以白粘痰 为主；常合并有电解质紊乱、肝肾功能损害；死因主要为心力衰渴和呼吸衰渴。 结论:慢性肺源性心脏病是慢性病理过程的结果，病情复杂，常合并有多脏器功 能改变。临床治疗应采取以抗感染、合理氧疗、加强营养和对症治疗为主的综合 措施，机械通气治疗日益受到重视。戒烟和预防上呼吸道感染是减少慢性肺源性 心脏病发病率和阻止病情发展的重要措施。 【关键词】慢性肺源性心脏病 慢性阻塞性肺疾病 呼吸衰竭 【中图分类号】R541.5 【文献标识码】A 【文章编号】1672-5085 (2014) 23-0014-02 Clinical Analysis of 66 cases with chronic pulmonary Heart disease Fang Qiufang Fuyang 2nd peoplersquo;s Hospital,An Hui 236015,China [Abstract] Objective To study clinical characteristics of chr onic pulm on ary heart disease and diagnosis and treatment methods according to clinical analysis of 66 cases with chr onic pulm on ary heart disease. Method Clin ical manifestatio nsjaboratory exami nations, adjuvant and treatme nt methods of 56 in patients from 2008,1 to 2013,12 were analyzed retrospectively. Results The main causes of chronic pulmonary heart disease were from chronic obstructive pulmonary disease ,multiple patients have a long history of smoking. The main clinical manifestations have fever,cough,white phlegm,dyspnea’vesicular rale cyanosis,double legs edema ,and so on. And always complicated with electrolyte distort,the most patients were with abnormal of hepatic and nephritic function ? The patients mainly died of heart failure and respiratory failure.Conclusions Chronic pulm on ary heart disease is result of chr onic pathological course and always complicated with multi organ disfunction.CIinical combined therapy methods in eludes an ti-bacteria, ratio nal oxyge n therapy, nu tritio n support and syn drome anagement’mechnical ventilation therapy was taken into more account than ever. Giving up smoking and preventing upper respiratory infection can reduce the morbidity of Chronic pulm on ary heart disease and prevent the conditions from deteriorati ng. 【Key words] Chronic pulmonary heart
