探讨床面高度作为摆位参数对放射治疗精确度的影响邙国荣何振了林承光中山大学肿瘤防治中心放疗科，广东，广州，510060【摘要】目的:探讨以床面高度作为垂直摆位参数，在放射治疗小可以减少因患者身体变化和固定装置等因素的影响引起的中心点的垂直位移偏差°材料与方法:将60例以真空袋固定的胸腹部肿瘤放射治疗患者在首次摆位时利川直线加速器屮的EP1D拍摄系统拍摄侧位片作为标准，并在放射治疗末段吋分别利用源皮距参数和床髙为摆位参数进行摆位并拍摄其侧位片，分别测虽图像中心点与首次摆位时的垂直位移变化，并比较不同方法Z间的中心点位移情况。结果：利用源皮距作为垂直定位基准时(A组),屮心点偏差3MM有28例,精确度为53.3% ；而采用以床面高度作为垂直定位基准时(B组)，中心点偏差3MM有10例，精确度为83.3%；B组的精确度较A组高(x 2 = 12. 478 , P0. 01 )。结论：在胸腹部放射治疗屮将床而高度作为垂肓摆位参数之一，可以减少中心垂肓移位偏基，提升摆位精准度, 使精确治疗的质量在放射治疗过程中得以保证，值得在临床工作中推广。【关键词】：放射疗法；床面高度；摆位参数；精确度The Effect of couch-height as Parameter of Vertical toRadiotherapy PrecisionKuang GuoRong , He ZhenYu , Lin ChengGuang?Dopartment of the Radiotherapy, CanccrCentcr, SunYat-scnUnivorsity, Guangzhou, 510060, P .R. China[Abstract】Objective: discuss that using couch-height as the parameter of vertical position in the radiotherapy reduce the vertical deviation from the central point, which is caused by the change of the patient s body, fixation and so on. Methods :At first time , 60 patients who were radiated on chest or abdomcn and fixed by Vacuum-bag were taken the side-position photo as the criterion by the Electronic Portal Image Device (EPTD) photograph system of 1inear accelerator. And last time we use respectively source-surface distance (SSD) and couch-height as the parameter to posit and take the side-position photos, then compare the vertical shifting from the contral point of the picture to the first-localion respectively. Result: Whenusing SSD as the benchmark of vertical orienlation (Group A), there are 28 cases which deviation from the central point aremore than 3MM and the precision is 53. 3%; While using couch-height (Group B), only 10 cases occurand the precision is 83- 3%. The precision of Group B is higher than that of Group A (x 2=12. 478 , P0. 01) Conclusion: The method using couch-height as the parameter of vertical position in the radiotherapy can reduce vertical deviation and enhance the precision of position.
