Instructions of NTU Health Exam for Exchange Students from China 臺灣大學交換學生健康檢查說明（中國地區姐妹校交換學生專用） In order to understand the general health condition of the new students, and to meet the regulations of National Taiwan University and our government’s requirements, all incoming exchange students should receive a health exam by a qualified physician. The registration procedure is not complete if the student does not have her/his health exam form completed. 臺大校方規定所有交換學生，須於入學前接受合格醫生所作的健康檢查，檢查 結果並須符合校方規定。未於申請時遞 交健康檢查報告者，將無法獲得入學許 可。 For exchange students’ convenience, you may take the health exam in your countries in recent 3 months as long as the items are included with the doctor’s signature and stamp of the hospital or clinic (for certification). 交換學生須於來臺交換前，提供近三個月內完成的健康檢查報告，並完成「學 生一般體格檢查表」上的所有檢查項目，該表格須有合格醫生的簽署，以及醫 院或是醫療檢驗所的正式戳章，以證明該檢查結果屬實。 You may download the health exam form from the website at .tw/healthexam/3-4.pdf and bring it to the hospital.
