All these patients were followedup for 6 months to 1 year. The average days, operation time, incidence of complicatio, mean blood loss in operation and the functional evaluation of hip joint were compared and analyzed. Results There was obvious difference between BHR 〔(73±19) min〕 and DHS groups 〔(96 ±33) min〕 (P). The rate of excellent and good in BHR group was %, which was higher than that of DHS group (%) (P). There were no difference between two groups in the incidence of complicatio (BHR group %, DHS group %), the mean hospitalization period 〔BHR group (34±26) d, DHS group (30±20) d〕 and the mean blood loss in operation〔BHR group (200±151) ml, DHS group (200 ±125) ml〕(all P).Conclusio The BHR with advantages of little complication, less bleeding, shorter operation time, and better effect, is one of the best alternative treatments to the aged patients with osteoporosic utable intertrochanteric fracture. 【Key words】 Utable intertrochanteric fracture; Dynamic hip screw (DHS); Bipolar hemiarthroplasty (BHR) 股骨粗隆间骨折约占全身骨折的%，既往多采用保守治疗，但保守治疗长期卧床并发症多，致残率和病死率高(35%左右)。而手术治疗由于病人可早期下床活动，避免了长期卧床带来的一系列问题，死亡率明显下降，治疗后功能恢复好于保守治疗。早期〔1，2〕手术已成为髋部骨折治疗的主要方法，但国内究竟选择内固定还是人工关节置换治疗老年不稳定型粗隆间骨折还存在争议〔3，4〕。进年来，大多学者〔4～7〕认为人工关节置换是理想的选择。本文通过回顾性研究，对本院治疗股骨粗隆间骨折的不同方法进行 总结 分析
