Methods The clinical data of 22 patients with complicated antler shape kidney stone from July 2009 to November 2011 . There were 13 male cases and 9 female cases , the age was 30 ~ 62 years old, Through the renal pelvis incision combined pneumatic lithotripsy for removal of calculus. Results The calculus was removed uneventfully in all patients，All of patients had no major bleeding and calyx injury, 3 patients had multiple lower poles caliceal stones, and Through the lower pole renal multiple lithotomy, 5 cases had residual stones, They were cured after ESWL. IVU was performed at the third month postoperatively , Kidney development is better than before, the stone disappear.
